主动电极在右室流出道间隔部起搏的应用

摘  要:目的:评价主动固定电极在右室流出道间隔部起搏的先进性。方法:33例患者,男12例,女21例;右室心尖部起搏9例,右室流出道间隔部起搏24例,观察电极置入时间,X线曝光时间和QRS波时限。电极置入后随访观察起搏阈值、阻抗、R波幅度,心功能情况。结果:主动固定电极组的置入时间和X线曝光时间均长于被动电极组(34.34±6.54 min vs 16.78±5.38 min,20.86±4.32 min vs 8.67±4.52 min);QRS波时限(0.14±0.04 s vs 0.16±0.03 s,P<0.01)。经过6个月随访,起搏阈值、阻抗两组比较差异无统计学意义,心功能改善。结论:主动固定电极植入优于被动电极植入。Objective: To evaluate the advancement of the application of active fixation leads with right ventricular septum pacing. Methods: 33 patients received permanent pacing, including 12 male and 21female, 9 patients pacing in right ventricular apex, and 24 patients pacing in right ventricular septum. Time for leads planting X - ray exposure time and the width of the QRS waves were recorded. Then pacing thresholds, impedances, R waves amplitude and Cardiac function were observed after the operation. Results:The group of active fixation leads took more time to plant leads and expose in X - ray than the one of Passive leads(34.34 ± 6.54 rain vs 16.78 ± 5.38 min, 20.86 ± 4.32 min vs 8.67 ± 4.52 rain), with the width of the QRS waves(0.14 ± 0.04 s vs 0.16 ± 0.03 s, P 〈 0.01 ). There was no significant difference in the pacing threshold and impedances between the two groups during 6 months, and cardiac function was improved. Conclusions: Active fixation leads is superior to passive leads.
