铜绿假单胞菌注射液预防胃癌术后腹腔复发与转移的研究

摘　　要：目的探讨铜绿假单胞菌注射液在体外对胃癌细胞杀伤作用,并观察其在进展期胃癌围手术期应用对预防术后腹腔复发与转移的作用。方法2005年1月至2007年12月上海交通大学医学院附属仁济医院普外科采用四甲基偶氮唑盐(MTT)法观察铜绿假单胞菌注射液抑制胃癌瘤株MKN45细胞增殖和杀伤作用的剂量依赖性;采用扫描电镜观察对胃癌瘤株MKN45的特异性结合。在此基础上选择T3M0进展期胃癌病人共60例,随机分为治疗组和对照组,观察其不良反应和对病人免疫学指标、术后腹腔复发及转移的影响。结果铜绿假单胞菌注射液能与胃癌瘤株MKN45细胞发生特异性结合并可进入细胞内抑制其增殖和生长,其杀伤作用随着用药浓度增加而加强。其安全性较好,用药组病人细胞免疫功能明显增强,腹膜复发和腹水脱落癌细胞阳性率显著低于对照组。结论铜绿假单胞菌注射液有直接杀伤胃癌瘤株MKN45细胞的作用,且这种作用存在剂量依赖性;作为腹腔免疫化疗重要手段,可以增强机体细胞免疫功能、有效预防进展期胃癌术后近期腹腔复发和转移。Objective To evaluate the killing effect by PA-MSHA vaccine on gastric cancer cells in vitro and its effect on the prophylaxis of post-operative intraperitoneal reeurrence and metastasis in advanced gastric cancer. Methods MTT method was used to observe the inhibition and killing effect by PA-MSHA vaccine on gastric caneer cell strain MKN45 between January 2005 and December 2007 at Renji Hospital Affiliated to Shanghai Jiao Tong University School of Medicine. Specifie combination and inhibition of cell proliferation were observed by transmission electron microscopy. Sixty cases of advanced gastric cancer T3M0 were selected into the study which were randomly separated into two groups： PA-MSHA group and control group, h observed side effects, effeet on the patients＇ immune function and intraperitoneal recurrence or metastasis. ResuLts PA-MSHA vaccine could specifically combine and penetrate into the gastric cancer cell strain MKN45 so as to inhibit its proliferation and the killing effect was in direct proportion to its （lose. The side effect of PA＇MSHA vaecine was comparatively limited. In PA-MSHA group, the cellular immune funetion was strengthened and intraperitoneal recurrence or metastasis decreased significantly. Conclusion PA-MSHA vaccine has dose-dependent killing effect on gastric cancer cell strain MKN45 by specific combination. It is safe and feasible which can strengthen the cellular immune function and reduce intraperitoneal recurrence or metastasis in advanced gastric cancer.
