液电碎石治疗对胆囊损伤的评价

摘  要:目的探讨液电碎石对胆囊壁的损伤程度和安全性。方法选择需要开腹行胆囊切除术的胆囊结石患者10例行放电电击试验,再行病理切片观察胆囊壁损伤情况。结果对分正对和平行两组,四档不同强度放电,病理切片观察,对其进行评分,并采用秩和试验统计分析得出不同强度放电对胆囊壁损伤有显著差异,不同方向放电对胆囊壁损伤有显著差异,具有统计学意义。结论液电碎石放电火花对胆囊壁有一定损伤,且不同档次放电,不同方位放电损伤不同,但并没有出现穿孔情况。只要按一定的操作要领,EHL是安全的有效的治疗方法。Objective To study the safety and degree of injury to gallbladder wall of electrohydraulic lithotripsy. Methods Giving an single-frequency electric shock gallbladder test on every 10 gallstone patients firstly who need an open choleeystectomy, then observing the gallbladder wall injury through pathological observation. Results 10 gallstone patients are divided into vertical group( E- lectrodes and gallbladder wall are closely vertical) and parallel group ( Electrodes and gallbladder wall are placed closely in the parallel) ,four different intensity discharge being given. By scoring through the pathological observation of gallbladder, the rank test statistical analysis results indicate that different intensities and directions of discharge on gallbladder wall injury have significant statistical differences. Conclusions EHL can leed to a certain degree of gallbladder wall injury, the degrees of injury depend on the intensity and direction of discharge. And no gallbladder perforation exists in EHL through pathological observation. As an result, EHL is a safe and effective therapy in hepatolithiasis according to a certain and necessary operation.
