摘  要:【目的】探讨高频超声对婴儿化脓性髋关节炎的诊断价值及临床意义。【方法】对本院骨科收治的14例资料完整的婴儿化脓性髋关节炎患儿的B超图像、X平片、MRI检查结果进行比较分析。【结果】各种影像学检查主要共同异常表现是关节积液、软组织肿胀、骨膜增厚及股骨头脱位等。①MRI显示关节积液最敏感,B超次之,两者无显著差异,明显优于X平片。②软组织肿胀亦为MRI最敏感,B超次之,③骨膜增厚或破坏以X线最敏感B超次之,④股骨头脱位以B超最敏感。⑤B超还能显示关节囊、关节滑膜增厚,软骨性股骨头变形等改变。【结论】超声检查简便快速、无侵袭性,在婴儿化脓性髋关节炎的诊断及治疗随访中具有重要意义。[Objective]To study the value of high-frequent ultrasound for the diagnosis of septic arthritis of the hip in infants and its clinical significance. [Methods]The B-ultrasound, X-ray and MR1 features and results in 14 infants with septic arthritis of the hip treated in our hospital were compared and analyzed. [Results] The main common abnormal appearances by these imaging examinations showed joint effusion, soft tissue swelling, periosteal thickening, dislocation of the femoral head and so on. MRI was the most sensitive to show the joint effusion and B-ultrasound was the second. There was no significant difference between the two examinations, and the two examinations were significantly better than the X ray. MRI was also the most sensitive to show soft tissue swelling and B-uhrasound was the second. X ray was the most sensitive to show the periosteal thickening or damage and B-ultrasound was the second. B-ultrasound was the most sensitive to show the dislocation of the femoral head and also showed joint capsule or synovial thickening, deformation of the femoral head cartilage and other changes. [Conclusion] Ultrasonography is simple, rapid and noninvasive, and has the significant value for the diagnosis, treatment and follow up of septic arthritis of the hip in infants.
