成人急性阑尾炎的超声诊断价值

摘　　要：目的探讨成人急性阑尾炎的超声诊断价值。方法运用高频及低频超声对167例成人急性阑尾炎患者进行检查,观察其声像图特征,并与术后病理结果对照,分析不同类型阑尾炎超声检查符合率及分型诊断符合率。结果不同类型急性阑尾炎超声显示率为96.4%,其声像图特征各异。超声检查分型诊断符合率:单纯型67.4%,化脓型88.4%,坏疽型86.0%,阑尾周围脓肿100%。单纯型与化脓型差异有统计学意义(P<0.05),单纯型与坏疽型差异有统计学意义(P<0.05),化脓型与坏疽型差异无统计学意义(P>0.05);高频超声符合率77.2%,低频超声符合率65.3%,联合应用高频与低频超声符合率86.8%,三者两两比较差别均有统计学意义(P<0.05)。结论超声检查经济、安全、无创、可重复性好,对成人急性阑尾炎的诊断及分型具有重要价值。Objective To investigate the diagnostic value of ultrasonography in adult acute appendicitis. Methods One hundred and sixty-seven cases of adult acute appendicitis were examined with high and low frequency ultrasonography. The features of sonogram were observed and compared with postoperative pathology, and the accordance rate of different types of ultrasonography and classified diagnosis was analyzed. Results The features of sonogram of different types of acute appendicitis which were depicted on ultrasonography in 161 （161/167, 96.4 % ） were various. The accordance rate of u ltrasonographic classification： 67.4% in simple appendicitis, 88.4% in acute purulent appendicitis, 86.0% in acute gangrenous appendicitis, and 100 % in periappendicular abscess was 100%. There were significant differences between simple appendicitis and acute purulent appendicitis and acute gangrenous appendicitis （P〈0.05）, but there was no difference between acute purulent appendicitis and acute gangrenous appendicitis （P〈0.05）. The accordance rate of different types of ultrasonography： 77.2% in high frequency ultrasonograpby, 65.3% in low frequency ultrasonography, and 86.8% in the combination. There was significant difference among the three groups （P〈0.05）. Conclusion Ultrasonography is economic, safe, noninvasive and repeatable, and it has great value in the diagnosis and classification of adult acute appendicitis.
