不同类型乳导管炎介入性药物灌洗治疗疗效比较

摘　　要：目的：探讨介入性药物冲洗结合保留灌注治疗乳腺导管炎的新方法及其在不同类型乳腺导管炎治疗中的疗效比较。方法：对经乳导管内视镜及实验室检查诊断的不同类型乳腺导管炎102例患者，应用乳腺导管针，通过溢液的乳孔直接插入乳腺导管，首先进行甲硝唑冲洗，然后再应用敏感抗生素加地塞米松进行保留灌注治疗。每日1次，3～5d为1疗程。结果：102例不同类型乳腺导管炎患者经1～2个疗程的治疗，95例（93．14％）治愈，其中扩张性导管炎44例（100％），出血性导管炎31例（96．88％），积乳性导管炎3例（60％），闭塞性导管炎17例（80．95％）；7例（6．86％）明显好转。95例治愈患者，随访2～6月均未再复发。所有患者均无不良反应发生。结论：介入性药物冲洗结合保留灌注治疗乳导管炎疗效好，尤其对扩张性导管炎和出血性导管炎治愈率高，具有很大的临床应用价值。Objective：To investigate the new method of interventional therapy with drug rinse and retained perfusion for mammary duct infection and compare the curative effect in different types of this lesion. Methods： The patients of mammary duct infection with nipple discharge diagnosed by breast fiberoptic endoscopy and laboratory tests in 102 cases were treated by interventional therapy with metronidazole rinse first,and then with sensitive antibiotics and dexamethasone retained perfusion using ductal needles through the discharging nipple pore directly into mammary duct. There was once per day and 3-5 days for a period. Results：Of 102 cases with different types of mammary duct infection, 95 cases （93.14%） were cured by one to two periods of treatment,included 44 cases（ 100 % ）with dilated mammary duct infection, 31 cases （96.88 % ） with hemorrhagic mammary duct infection, 3 cases （60 %） with accumulate milk duct infection, 17 cases （80.95 %） with occlusive duct infection, and the rest 7 cases （6.86%）also improved obviously. Ninty five cases （93. 14%）, who were cured, were not recur in the following 2 6 months. There was no adverse reaction in all patients. Conclusion： The treatment of mammary duct infection with interventional drug rinse and retained perfusion has good curative effect and high cure rate,especially to dilated mammary duct infection and hemorrhagic duct infection, and has great value of clinical application.
