摘　　要：目的:探讨头皮撕脱伤后颅骨外露的简易治疗方法,尤其针对经济条件较差的患者群。方法:2003年9月～2008年4月,5例头皮大面积撕脱伤颅骨外露患者,因血管吻合后头皮坏死,头皮缺损面积为25cm×33cm～16cm×21cm,颅骨外露面积6cm×3cm～14cm×20cm,颅骨外露部位采用颌面外科磨骨器械",井"字形磨除颅骨外板,表面移植中厚皮片,创面加压包扎。结果:所有患者均未发生感染、大出血等并发症,除1例外,头皮裸露创面移植皮片存活良好,颅骨外露部位无移植皮片坏死,术后随访3～6月,外观未见明显异常。结论:对于大面积头皮撕脱伤合并颅骨外露患者,直接颅骨磨骨植皮,可尽早修复外露颅骨创面,方法简单易行,对经济条件较差的患者群尤为适用。Objective To investigate the simple method for cranium exposure after scalp avulsioin,especial for those poor patients.Methods From Sep.2003 to Apr.2008,5 patients with cranium exposure after scalp avulsioin,who received unsuccessful blood vessels anastomosis,scalp defect area from 25cm×33cm-16cm×21cm,cranium exposure area from 6cm×3cm-14cm×20cm,were treated with ＇well＇ shape cranium grinding and skin grafting.The wounds were bound up with pressure.Results In all 5 patients,there were no other complication such as infection and severe bleeding.Skin graft survived well in 4 patients except 1 patient,After 3-6 months follow-up,all of the patients showed good appearance and were satisfied with the operation results.Conclusion For those poor patients with cranium exposure after scalp avulsion,cranium grinding and skin grafting is a simple and easy method to cover cranium exposure wound.
