聚丙烯酰胺水凝胶注射隆乳术后切取方法及同期假体隆乳的探讨

摘　　要：目的:探讨聚丙烯酰胺水凝胶注射隆乳切开取出方法和同期假体隆乳的适应证。方法:在乳房彩超提示下,经腋窝或乳晕切口取出乳房内水凝胶和变性坏死组织,在受植腔良好情况下同时植入硅凝胶假体隆乳。结果:本组189例,术后随访3个月～5年6个月,注射隆乳并发症消失,同期假体隆乳156例术后乳房形状好、手感软。结论:针对水凝胶注射隆乳并发症的不同情况,选择腋窝或乳晕切口能最大限度取出乳房内水凝胶,尤其腋窝切口更隐蔽,应首选;对乳房受植腔良好者,同期置入假体隆乳安全可行、效果好。Objective To discuss the method for taking out the injected polyacrylamide hydrogel for breast augmentation and the indications of simultaneous breast implantation. Methods Under the guidance of color Doppler ultrasound, take out the hydrophilic gel and necrotic tissue through the incision on armpit or mammary areola,and immediately implant the silicone gel breast implant when the recipient bed is good condision. Results Among the 189 cases of this group,there are 156 cases in which the complicasions of breast augmentation by injection have disappeared according to the post-operative follow-up visits （3 months-5 years and 6 months） and the corresponding breast implantation has provided good shape and soft feeling. Conclusion With respect to the different complications resulted from the injection of hydrophilic gel for breast augmentation,the incision on armpit or mammary areola can help remove the hydrophilic gel in the breast to the maximum extent.In particular,the incision on the armpit can be well concealed and should be the first choice.For those with recipient bed in good condition,the simultaneous implantation of breast implant will provide good safety and satisfactory effect.
