摘  要:目的探讨天晴宁(羟乙基淀粉130/0.4氯化钠)注射液快速扩容时对血液流变学的影响。方法择期脊柱手术患者40例,随机分为2组,A组为实验组(20例),B组为对照组(20例)。用Graseby输液泵分别输注天晴宁注射液和复方氯化钠注射液,速率均为20mL/(kg.h),分别于输液前、输液后30min和输液后60min时,经上肢静脉采血3mL,肝素抗凝,送血流变检验室检测。结果输液后,两组患者红细胞压积均显著下降(P<0.05),全血粘度(高切、中切和低切)均显著降低(P<0.05),但A组的中、低切全血粘度比B组下降更为明显(P<0.05),血浆粘度和红细胞聚集指数无显著性变化(P>0.05)。结论天晴宁注射液在降低全血粘度,改善血液流变学状态、防止血栓形成等方面优于复方氯化钠,是一种较安全的血浆容量扩张剂。Objective To investigate the hemorheological effects of acute hypervolemic hemodilution with TIANQINGNING injection. Methods 40 ASA Ⅰ - Ⅱpatients scheduled for elective operation on spine were randomly divided into 2 groups. TIANQINGNING injection infusion rate was 20 mL/( kg · h)in Group A, the control group ( B ) received Ringer' solution at a rate of 20 mL/( kg · h). The venous blood samples were taken for rheological analysis in the 2 groups before and 30 min,60 min after infusion. Results Hematocrit and blood viscosity decreased significantly( P 〈 0. 05 )in the 2 groups after infusion. No significant change was discovered in plasma viscosity and erythrocyte aggregation index( P 〉 0.05 )in the 2 groups after infusion. Conclusion TIANQINGNING injection can significantly improve blood rheology, and it is beneficial for the microcirculatory perfusion.
