DHS内固定治疗股骨粗隆间骨折的临床疗效评价

摘　　要：目的评价动力加压髋螺钉（DHS）内固定治疗股骨粗隆间骨折的临床疗效。方法对2004年7月—2008年6月87例股骨粗隆间骨折的患者采用动力髋螺钉治疗，对手术方法及疗效回顾分析。结果所有患者均获得随访，随访时间6—18个月，平均随访时间9．2个月；术后按Harris评定标准，髋关节功能优良率为90．8％；术后出现3例髋内翻，2例延迟愈合，无1例死亡，无切口感染、深静脉血栓形成、骨折不愈合及内固定断裂。结论DHS手术内固定是治疗股骨粗隆间骨折的一种有效方法，强调严格的手术适应证、充分的术前准备、规范的DHS手术操作技巧、个性化的术后功能锻炼等的重要性。Objective To evaluate the clinical curative effect of the treatment of intertrochanteric fractures with internal fixation by DHS. Methods 87 eases of intertrochanterie fractures-were treated by DHS, and to proceed retrospective analysis of technique and curative effect. Results All patients were followed up for 6 to 18 months after the operation, with an average of 9.2 months. According to Harris＇ evaluation standard, the excellent and good rate of postoperative hip joint function was 90.8%. In these following up cases, coxa vara was found in 3 cases, delayed union was in 2 cases. No death,no postoperative infection,no deep venous thrombosis,and no fracture nonunion or internal fixation breakage exist. Conclusions Internal fixation by DHS was an effective method of treatment of intertrochanteric fractures. It is important to emphasize significance of strict operative indication, sufficient preoperotive preparation, normative operative procedure skill, and adaptive postoperative functional exercise.
