血清基质金属蛋白酶与不同类型心绞痛病人近期预后的关系

摘　　要：目的:探讨未接受介入治疗的不同类型心绞痛病人血清基质金属蛋白酶-9(MMP-9)、基质金属蛋白酶组织抑制因子-1(TIMP-1)水平的变化与近期预后的关系。方法:46例不稳定型心绞痛病人为UAP组,36例稳定型心绞痛病人为SAP组,30名体检健康者为对照组。比较各组血清MMP-9和TIMP-1水平变化;随访3mo主要心血管事件。结果:UAP组入院第1d MMP-9水平高于SAP组(P>0.05),前者持续在第3d(272.23±20.09 vs 165.57±22.34)ng/mL、第7d继续升高(337.35±53.12 vs 125.22±27.25)ng/mL,后者则持续下降(P<0.05);TIMP-1表现同样的变化趋势。3mo随访期间发生心脏事件33例,无心脏事件49例。有心脏事件入院第1、3、7d血清MMP-9水平高于无心脏事件(P<0.05)。结论:UAP病人MMP-9、TIMP-1血清水平动态升高,与近期心血管事件有关。MMP-9、TIMP-1可作为不同类型心绞痛病人危险分层的指标之一。Objective：To study the serum level of matrix metalloproteinase-9（MMP-9）,tissue inhibitors of matrix metalloproteinase-1（TIMP-1）and its short-term prognosticvalue for patients with angina pectoris（AP）.Methods：46 cases of patients with unstable angina pectoris（UAP） and 36 cases of patients with stable angina pectoris（SAP）.While 30 cases of healthy individuals were selected as control group.The serum levels of MMP-9 and TIMP-1 were compared.Results：Serum levels of MMP-9 were significantly higher at 3rd day（272.23±20.09 vs 165.57±22.34）ng/mL、and 7th day（337.35±53.12 vs 125.22±27.25）ng/mL in UAP group than those in the SAP group（P〈0.05）.In the follow-up 3 months,cardiac events group（n=33） had higher serum level of MMP-9 than those of no cardiac events group（n=49）（P〈0.05）.Conclusion：The serum level of MMP-9 plays important role in occurrence of cardiac events and short-term prognostic.It could be one index of cardiac events.
