肿瘤放疗科住院患者抗菌药物使用调查

摘　　要：目的 了解肿瘤放疗科住院患者抗菌药物的使用情况。方法对本院肿瘤放疗科103例住院患者抗菌药物的应用情况进行调查，采用横截面研究方法。结果抗菌药物的使用率35．9％，M1期的肿瘤患者应用率64．9％，较MO期患者的抗菌药应用率（19．7％）明显增加（P〈0．01）。以治疗性用药为主占83．8％，预防用药在住院患者中的总使用率为5．8％；单用药占62．2％；主要使用的抗菌药物是头孢曲松钠、阿莫西彬克拉维酸钾、五水头孢唑啉、甲硝唑；用药途径主要以静脉为主，占86．5％；细菌标本送检率仅为29．7％。结论严格掌握抗菌药物的适应证，减少不必要的预防性用药；重视肿瘤治疗对于患者免疫功能的影响；食管癌瘤灶放疗周围低剂量的区域易出现感染，值得临床重视和预防。另外，提高标本培养的送检率对于抗菌药物的合理应用十分重要。Objective To investigate antimicrobial agents used in inpatients in department of radiation oncology. Method Prevalence survey was made to analyze the usage rate of antimicrobial agent in 103 inpatients in department of radiation oncology of a cancer hospital. Results Percent of antimicrobial agent using was 35.9 % ;The percent of antimicrobial agent using in M1 phase was higher than that in M0 phase at the same time P 〈 0. 01 （ 64. 9% vs 19.7% ） ; Antimicrobial agents were mainly used for treatment（ 83.8% ） ; Preventive usage rate in all the inpatients surveyed was 5.8% ; Single application rate was 62. 2% ; Mainly through intravenous route （86. 5% ）;The most frequently used antimicrobial agents were Ceftriaxion, Amoxicillin/clavulanate potassium,Cefazolin sodium pentahydrate, Metronidazole; The bacterial examination rate was only 29. 7%. Conclusions More attention needs to be paid to disinfect wards, especially in the M1 phase＇s ; keeping a tight antimicrobial regime could reduce the usage rate of antimicrobial agent ; information on immune function after tumor treatment is needed ; The regions around the esophageal carcinoma of low - dose radiation are prone to infection ; increasing bacterial examination rate is important for appropriate using of antimicrobial agents.
