摘  要:目的:探讨简化LHRH(促性腺激素释放激素)刺激实验在儿童性早熟疾病鉴别诊断中的价值。方法:随机选择性早熟病例12例进行传统LHRH刺激试验(五点位法),并于静脉应用LHRH药物40 min时取静脉血,测定0、30、40、60、90、120 min 6个点位的LH、FSH值,分别比较确定0、40 min两个点位(简称两点位法)的LH峰值及LH/FSH比值;0、30、60、90 min 4个点位(简称四点位法)的LH峰值及LH/FSH比值;0、30、60、90、120 min 5个点位(简称五点位法)的LH峰值及LH/FSH比值。每例患者均分别以上述3种不同点位法得到的LH峰值及LH/FSH比值判断性早熟的类型,并分别将两点位法与五点位法、四点位法与五点位法LHRH刺激试验得到的试验结果进行比较,进行统计学处理。结果:以两点位法与五点位法判断的性早熟类型比较,差异无统计学意义(χ2=3.2,P>0.05);以四点位法与五点位法判断的性早熟类型比较,差异无统计学意义(χ2=0.22,P>0.05)。结论:简化LHRH刺激实验在儿童性早熟的鉴别诊断,尤其是年幼儿童性早熟的鉴别中具有重要价值。Objective:To explore the value of simplified luteinizing hormone releasing hormone(LHRH) stimulation in differential diagnosis of precocious puberty.Methods:12 children with precocious puberty underwent traditional LHRH stimulation(5 time-points),then venous blood samples were taken at 0,30,40,60,90 and 120 minutes after LHRH to detect the levels of LH and FSH,peak value of LH and ratio of LH/FSH were compared at different time points including two time-points(0 and 40 minutes),four time-points(0,30,60 and 90 minutes) and five time-points(0,30,60,90 and 120 minutes).The types of precocious puberty were determined according to peak value of LH and ratio of LH/FSH,and the results were compared among different methods.Results:There was no significant difference in types of precocious puberty between two time-points and five time-points as well as between four time-points and five time-points(P〉0.05).Conclusion:Simplified LHRH stimulation plays an important role in differential diagnosis of precocious puberty.
