摘　　要：目的探讨老年腹股沟疝的治疗策略。方法回顾分析老年腹股沟疝治疗的临床数据，手术分为2组进行对照，比较择期手术组和急诊手术组的经济学指标。结果择期手术病人共326例，平均年龄（74．00±5．83）岁；急诊手术组26例，平均年龄（79．96±6．43）岁。住院时间择期手术组平均（8．37±3．93）d，急诊手术组平均（10．65±8．69）d；住院费用择期手术组平均（5918．40±2506．29）元，急诊手术组平均（10147．58±7682．10）元。择期手术组与急诊手术组住院时间差异无统计学意义（P=0．1562）；择期手术组住院费用显著低于急诊手术组（P=0．0002），差异有统计学意义。结论老年腹股沟疝手术安全性较高，择期手术组的社会经济学指标明显优于急诊手术组。老年腹股沟疝患者应当选择积极外科早期治疗。Objective To explore the strategy of treatment for inguinal hernia in senior patients. Methods Clinical data of inguinal hernia in senior patients were analyzed retrospectively to compare the economic index between the selective surgery group and the emergency surgery group. Results The total hospital expense in the selective surgery group was significantly less than that in the emergency surgery group （P=0.0002）. There was no significant difference in the length ofhospital stay between the two groups （P=0.1562）. Conclusions Surgical management of inguinal hernia is safe in senior patients. The social economic index in the selective surgery group was more favorable than that in the emergency surgery group. Early selective surgery is the optimal choice for the treatment of inguinal hernia in senior patients.
