TNF-α、IL-10在妊娠期高血压疾病患者血清中的表达及平衡变化

摘　　要：目的通过观察妊娠期高血压疾病患者血清TNF-α(Th1型细胞因子)、IL-10(Th2型细胞因子)水平及TNF-α/IL-10(Th1/Th2)比值的变化,来探讨Th1、Th2型细胞因子在妊娠期高血压疾病发病中所起的作用。方法采用酶免疫化学发光法测定58例妊娠期高血压疾病孕妇(其中妊娠期高血压23例、轻度子痫前期19例、重度子痫前期16例)和30例正常孕妇(对照组)的TNF-α、IL-10水平及计算TNF-α/IL-10比值并进行比较。结果TNF-α在妊娠期高血压疾病组血清中水平明显高于对照组,差异有统计学意义,且妊娠期高血压疾病不同分类间差异也有显著性,4组间IL-10水平差异无显著性,妊娠期高血压疾病组TNF-α/IL-10比值显著高于对照组,且各组间差异也有显著性。结论TNF-α参与了妊娠期高血压疾病的发病进程,血清Th1/Th2平衡失调是妊娠期高血压疾病的重要病理生理变化,即免疫失衡在妊娠期高血压疾病的发生发展中起着重要的作用。Objective To investigate the function of Th1- and Th2-type cytokines in hypertensive disorder complicating pregnancy by measuring serum TNF-α（Th 1-type cytokines）,IL-10 （Th2-type cytokines）and the ratio of TNF-αand IL-10（Th1/Th2）. Methods We compared serum TNF-α, IL-10 and the ratio of TNF-aand IL-10 be- tween 58 women with hypertensive disorder complicating pregnancy（23 gestational hypertension, 19 mild preeclampsia, 16 severe preeclampsia） and 30 normal pregnant women by Chemiluminescent Enzyme Immunoassay. Results Serum TNF-αand the ratio of TNF-αand IL-10 in hypertensive disorder complicating pregnancy group was significantly higher than that in normal pregnant women group,and there was significant difference among hypertensive disorder complicating pregnancy of different degree.There was no significant difference between the two groups on serum IL- 10. Conclusion TNF-αis involved in the progress of hypertensive disorder complicating pregnancy,and the imbalance of serum Th1/Th2 is an important pathoghysiology change ,which indicats that immunity imbalance plays an important role in hypertensive disorder complicating pregnancy.
