64层螺旋CT在急诊2型糖尿病伴胸痛患者中的应用研究

摘　　要：目的探讨64层螺旋CT(MSCT)冠状动脉造影在急诊2型糖尿病伴胸痛患者临床应用价值。方法对50例急诊2型糖尿病伴冠心病患者行64层MSCT冠脉造影检查,利用其先进的后处理功能对冠脉进行重组,以显示冠脉各主支及分支,并对冠脉病变进行诊断。结果50例行64层MSCT冠脉造影均成功,显示冠脉各主支(左冠脉主支、左冠脉回旋支、左冠脉前降支及右冠脉主支)及其分支(包括右冠脉的后降支、左室后支、圆锥支、窦房结动脉及左冠脉的对角支、钝缘支等),98例患者的208支冠脉有不同程度的硬斑块和软斑块,其中88例的128支冠脉伴有不同程度的狭窄,88例病员同时行选择性血管造影的患者显示125支冠脉有不同程度的狭窄。结论64层MSCT是一种安全可靠的检查方法,在急诊2型糖尿病伴胸痛患者中具有广阔的应用前景。Objective To study the clinical application of 64-slices spiral CT in coronary artery in type 2 diabetics with coronary heart disease in emergency department. Methods Fifty-two patients were performed 64-slices spiral CT in coronary artery, 26 of the 52 patients underwent selective coronary artery angiography. Results The main branches and part of the sub-branches of the coronary artery were clearly displayed. 108 coronary artery branches of 46 patients were found stenosis.In 28 cases with conventional angiography, all main branches and parts of sub-branches of coronary artery were displayed clearly. All patients demonstrated coronary artery stenosis. The diagnostic accuracy of 64-slices spiral CT was 94.12%. Conclusion As a minimally invasive examination, 64-slices spiral CT is a valuable method to detect and diagnosed the diseases of coronary artery in type 2 diabetes with coronary heart disease in emergency department.
