超声对乳腺癌的诊断价值

摘　　要：目的 探讨超声对乳腺癌的诊断意义和肿瘤声像图的特征。方法回顾性分析263例乳腺癌患者的声像图，并与X线钼靶摄影、MRI检查作比较。结果超声对乳腺癌的检出率为87．45％（230／263）。声像图特征表现为低回声的单个病灶为多，占96．58％（254／263）；肿块边缘不规则，轮廓模糊，呈毛刺状或蟹足征达86．69％（228／263）；边界整齐，轮廓清楚，但无包膜为13．31％（35／263）；肿块纵横〉1占90．49％（238／263）；肿块内可见强回声斑点反射90．87％（239／263）；77．56％（204／263）的患者伴有同侧腋窝下淋巴结肿大。X线钼靶摄影对乳腺癌的检出率为96．74％（208／215）、MRI则为97．92％（47／48）均明显高于超声（P〈0．01或〈0．05）。结论虽然超声对乳腺癌的检出率不及X线钼靶摄影和MRI的高，但由于超声具有无创伤、无放射性、费用低等的特点，所以仍是乳腺癌的首选普查方法。Objective To probe into a meaning of ultrasonic diagnosis for galactophore cancer and a character of cancer ultrasonic image. Methods To analyze retrospectively 263 ultrasonic images with galactophore cancer, and to contrast with X molybdenum target photograph, and MRI. Results The discovering rate of ultrasonic for galactophore cacer was 87.45% （230/263）. The ultrasonic images were numerous low echo of singleness focus, they were 96. 58% （254/263） ,the edge of tumours were abnormity, the contours were faintness, they were like burr or crab legs （86. 69% ） （228/263） ;the edges were in order, the contours were in clarity but no envelopes 13.31% （35/263） ;the tumors in length and breadth 〉 1 were 90.49% （238/263） ;strong echo flecks in the tumors were 90. 87% （239/263） ; the lymph node of armpit in 77. 56% （204/263） patients were intumesce. The detection rate of galactophore cancer in X molybdenum target photograph was 96.74% （208/ 215） ,and MRI was 97. 92% （47/48） ,they were all higher B ultrasonic （P 〈0.01 or 〈0. 05）. Conclusion The detection rate of galactophore cancer in ultrasonic is not as good as X molybdenum target photograph and MRI, but the ultrasonic is no-hurt, no-radioactivity and charge low so that it is a first way of galactophore cancer examination.
