中医中药对术后尿潴留家兔膀胱功能恢复的影响

摘　　要：目的探讨中医中药对家兔神经损伤性尿潴留的作用。方法将45只家兔分为模型组、实验组、正常对照组,每组15只,实验组术后予中药及针灸治疗,余两组术后不做特殊处理。4d后分别进行排尿阈值及残余尿量测定,并用免疫组织化学和图像分析技术半定量测定各组膀胱逼尿肌胆碱能受体的表达。结果实验组膀胱内压较治疗前明显增高,差异具有统计学意义(P<0.05),且高于模型组膀胱内压(P<0.05)。实验组排尿阈值、残余尿量明显低于模型组(P<0.05)。膀胱逼尿肌胆碱能受体表达,实验组高于模型组(P<0.05)。结论中医药疗法可以提高神经损伤性尿潴留家兔膀胱逼尿肌胆碱能受体的表达,提高膀胱压力,降低排尿阈值和残余尿量,促进术后膀胱功能恢复。Objective To explore the effect of traditional Chinese medicine on nerve injury induced urinary retention in rabbit model, Methods 45 rabbits were divided into model group, experimental group and normal control group, 15 rabbits in each one. Chinese herbs and acupuncture were applied in experimental group after operation. No specific management was administered in the rest two groups after operation. Urination threshold and residual urine volume were measured 4 days later, immunohistochemistry and image semi - quantitative analysis technique were applied to determine the expression of cholinergic receptors in bladder detrusor muscle in each group. Results After treatment, in experimental group, the bladder pressure increased apparently compared with that before treatment, indicating statistical significant difference（ P 〈 0.05 ） and higher than that in model group（ P 〈 0.05 ）. The urination threshold of experimental group was significantly lower than that of model group（ P 〈 0.05 ）. The residual urine volume of experimental group was significantly lower than that of model group（ P 〈 0, 05 ）. The expression of cholinergic receptors in bladder detrusor muscle in experimental group was higher than that in model group （P 〈 0.05 ）. Conclusion Traditional Chinese medicine can improve the expression of cholinergic receptor in bladder detrusor muscle, increase bladder pressure, reduce urination threshold and residual urine volume and promote the postoperative bladder recovery of nerve injury induced urinary retention in rabbits.
