盐酸二甲双胍对老年糖尿病患者黎明现象的影响

摘　　要：目的使用动态血糖监测系统观察老年2型糖尿病患者的黎明现象及盐酸二甲双胍治疗后黎明现象的改善状况。方法对6例临床判断有黎明现象的老年2型糖尿病患者佩戴实时动态血糖监测系统(GuardianR RTCGMS),给予盐酸二甲双胍治疗,观察黎明现象的改善情况。结果动态血糖监测显示6例患者均存在黎明现象,给予盐酸二甲双胍0.5g,2/d治疗,黎明现象得到明显改善,平均血糖水平从8.1±1.5mmol/L降至6.6±1.0mmol/L(P<0.01);日内平均血糖波动幅度从2.7±2.0mmol/L降至1.0±0.6mmol/L(P<0.01)。血糖≥8.0mmol/L所占的时间百分比由59.2%降至25.5%(P<0.05),3.9mmol/L<血糖<8.0mmol/L所占的时间百分比由39.7%增至73.2%(P<0.05)。结论盐酸二甲双胍可以有效地改善部分老年糖尿病患者的黎明现象,实时动态血糖监测有助于老年2型糖尿病患者黎明现象的诊断和治疗效果的观察。Objective To observe early morning hyperglycaemia＂dawn phenomenom＂in type 2 elderly diabetes mellitus with Real-Time continuous glucose monitoring and assessment of curative effect by Metformin Hydrochloride. Methods Six type 2 elder diabetes patients with dawn phenomenom was measured by Guardian RT CGMS before Metformins Hydrochloride were taken, assessment of curative effect before and after Mefformin therapy. Results The dawn phenomenom were observed in patients by CGMS. Mefformin Hydrochloride,0.5g,twice a day were given and the dawn phenomenon were improved significantly. The mean blood glucose of the whole day were decreased from 8.1±1.5 mmol/L to 6.6±1.0 mmol/L（P〈0.05）. The mean amplitude of glycemic fluctuation were decreased from 2.7＋2.0 mmol/L to 1.0±0.6 mmol/L（P〈0.05）. The percentage of TBG ≥8.0 mmol/L were reduced from 59.2% to 25.5%（P〈0.05）. The percentage of 3.9 mmol/L〈TBG〈8.0 mmol/L were increased from 39.7% to 73.2%（P〈 0.05 ）. Conclusions Mefformin Hydrochloride can effectively improve the ＂dawn phenomenom＂in some type 2 elderly diabetes patients and the Guardian RT CGMS profile provides informations useful for the diagnosis of clawn phenomenom and assessment of curative effect.
