糖尿病肾病患者炎症相关因子水平检测的临床意义

摘　　要：目的探讨糖尿病肾病(diabetic kidney disease,DKD)患者的血清C反应蛋白(CRP)、可溶性白细胞介素-2受体(sIL-2R)、白细胞介素-6(IL-6)和血清转化生长因子-β1(TGF-β1)水平的变化在肾病发生、发展中的作用。方法将95例DKD患者根据24h尿微量白蛋白排泄率(UAER)分为:无蛋白尿组(n=31)、微量蛋白尿组(n=34)和大量蛋白尿组(n=30)。观察各组患者CRP、sIL-2R、IL-6和TGF-β1水平的变化,并与正常对照组(n=30)进行比较。结果无蛋白尿组、微量蛋白尿组及大量蛋白尿组的CRP、sIL-2R、IL-6及TGF-β1水平均明显高于健康对照组(P<0.01);微量蛋白尿组的CRP、sIL-2R、IL-6及TGF-β1水平明显高于无蛋白尿组(P<0.05);大量蛋白尿组的CRP、sIL-2R、IL-6及TGF-β1水平明显高于微量蛋白尿组(P<0.01)。结论早期检测血清CRP、sIL-2R、IL-6及TGF-β1水平的变化可以作为临床观察DKD病情及判断糖尿病预后的参考指标。Objective To investigate the significance of changes of serum C-reactive protein （CRP） ,interleukin-2 receptor （SIL-2R）, interleukin-6 （IL-6） and transforming growth factor-beta 1 （TGF-β1 ）levels in the patients with diabetic kidney disease. Methods Serum CRP,SIL-2R,IL-6 and TGF-β1 levels were measured with RIA in 95 patients with type2 DM and 30 controls. The 95 diabetic patients devided into three groups based urine albumin excretion rate. Group A： without alblminuria （UAER〈20 ug/ min, n=31 ） , Group B ： micro-albuminuria （ UAER〈200 ug/min, n=34 ） , Group C ： macro-albuminuria （UAER〉200 ug/min, n=30）. Results Serum levels of CRP,SIL-2R,IL-6 and TGF-β1 were significantly higher in all the patients with type 2 diabetes than those in controls （P〈0.01）. Serum levels of CRP,SIL-2R,IL-6 and TGF-β1 significantly higher in DM patients with microalbumin uria than those in patients without albuminuria（P〈0.05）. Serum levels of CRP,SIL-2R,IL-6 and TGF-β1 were significantly higher in diabetic patients with macroalbuminuria than those in patients with microalbuminuria （P〈0.01）. Conclusions Determination of serum CRP,SIL-2R,IL-6 and TGF-β1 levels might be helpful for early detection of diabetic kidney disease and monitoring the progress of the disease process.
