摘  要:目的:探索64层螺旋CT对冠脉管腔的可评价率和诊断不同程度冠脉狭窄的敏感性、特异性、阳性预测值和阴性预测值并进行比较。方法:将我院2006年1月~2008年7月经选择性冠状动脉造影(selective coronary angiography,SCA)对比的127例64层螺旋CT检测的冠脉狭窄患者的资料进行整理、统计、分析和对比。结果:冠脉管腔可评价率为95.6%。轻、中、重度冠脉狭窄的敏感性为85.9%、90.3%、95.3%(P<0.05);特异性为98.3%、99.0%、99.5%(P<0.05);阳性预测值为87.8%、91.6%、93.5%(P>0.05);阴性预测值为98.0%、99.0%、99.7%(P<0.001)。结论:64层CT对冠脉管腔的可评价率高,对诊断轻、中、重度冠脉狭窄的敏感性、特异性、阳性预测值、阴性预测值均较高,且各百分率有随冠脉狭窄程度的增加而逐渐增高的趋势,并且发现排除性指标优于诊断性指标。Objective:To explore the evaluation of the performance of 64-slice spiral CT(SCT) in the coronary artery imaging and to compare the sensitivity,the specificity,the positive predictive value,the negative predictive value of diagnosis variable degrees of coronary artery stenosis.Methods:One hundred and twenty seven patients with suspected coronary artery disease underwent contrast enhanced 64SCT and conventional selective coronary angiography,the outcomes of the two kinds of examination were analysed and compared.Resuits:95.6% coronary arteries of all could be assessed. The sensitivity, specificity, positive and negative predictive values in 64SCT prediction in the mild, moderate, and severe coronary artery stenosis were 85.9%, 90.3%, 95.3% ( P 〈 0. 05), 98.3%, 99.0%, 99.5%( P 〈0.05), 87.8%, 91.6%, 93.5%( P 〉0.05), 98.0%, 99.0%, 99.7%( P 〈0.001), respectively. Conclusion: 64SCT could well display the coronary vessel, both the sensitivity, specificity, positive and negative predictive values in 64SCT prediction in the mild, moderate, and severe coronary artery stenosis were high, and the percentage increa.sed while the degree of coronary artery stenosis increased.
