摘 要:目的:观察ω-3鱼油脂肪乳对胃肠道肿瘤病人术后细胞免疫功能和炎性反应的影响。方法:42胃肠道肿瘤病人随机分为对照组(20例)和研究组(22例)。对照组病人给予低热量PN治疗,而研究组加用ω-3鱼油脂肪乳。术后第1天起,连续5 d接受等氮、等热量的PN支持。观察病人术前、术后第1天和第6天外周血中T淋巴细胞亚群(CD3+、CD4+、CD8+、CD4+/CD8+比值)和血清C反应蛋白(CRP)的变化,并进行统计分析。结果:两组病人术后第1天CD3+、CD4+和CD4+/CD8+比值均有所下降,术后第6天研究组CD3+、CD4+和CD4+/CD8+比值上升,而对照组上升不明显。两组病人术后第1天CRP水平明显上升,术后第6天又明显下降,但仍高于术前水平,且研究组较对照组低。结论:胃肠道肿瘤病人术后早期应用ω-3鱼油脂肪乳能改善病人细胞免疫功能,抑制机体炎性反应,促进病人术后康复。Objective : To investigate the effect of ω-3 fish oil on immunologic function and inflammatory reaction of patients with gastrointestinal tumor after operation. Methods: 42 cases with gastrointestinal tumor were divided into control group and study group. The patients in control group were treated with hy- pocalorie parenteral nutrition, and those in study group received ω-3 fatty acids additionally. The total par- enteral nutrition was transfused from the first day to the fifth day after operation. CD3 + , CD4 + , CD8 + , CD4 +/CD8 + and CRP of the two groups were detected on the day before the operation, the first and sixth day after operation. Results : CD3 + , CD4 + , CD8 + ter operation, and increased study group. The CRP level after five days of parenteral of the two groups increased CD4 +/CD8 + of the two groups were dropped af- nutrition..However, the increase was obviously in obviously affbr operation but it also dropped obvi-ously after five days TPN support especially in the study group. Conclusion : Early total parenteral nu- trition contained ω-3 fatty acids can improve immunologic function and control inflammatory reaction. It may have beneficial effect on the recovery of patients with gastrointestinal tumor after operation.
