糖尿病视网膜病变患者脂联素和肿瘤坏死因子-α、可溶性细胞间黏附分子-1、超敏C-反应蛋白相关性分析

摘　　要：目的:探讨2型糖尿病患者血清脂联素、肿瘤坏死因子-α、可溶性细胞间黏附分子-1、超敏C-反应蛋白在2型糖尿病及视网膜病变发生、发展中的作用。方法:检测35例糖尿病视网膜病变患者(DR组)脂联素、肿瘤坏死因子-α、可溶性细胞间黏附分子-1、超敏C-反应蛋白,并与40例未并发糖尿病视网膜病变的2型糖尿病患者(NDR组)和50例正常对照组(NC组)比较。结果:2型糖尿病组患者肿瘤坏死因子-α、可溶性细胞间黏附分子-1、超敏C-反应蛋白水平均明显高于正常对照组(P<0.01),脂联素水平明显低于正常对照组(P<0.01);DR组肿瘤坏死因子-α、可溶性细胞间黏附分子-1、超敏C-反应蛋白水平明显高于NDR组(P<0.01),脂联素水平明显低于NDR组(P<0.01)。血清脂联素和肿瘤坏死因子-α、可溶性细胞间黏附分子-1、超敏C-反应蛋白呈显著负相关(r=-0.512,r=-0.517,r=-0.523)。结论:脂联素具有抗高血糖、抗炎等多种效应。脂联素降低,肿瘤坏死因子-α、可溶性细胞间黏附分子-1、超敏C-反应蛋白升高在2型糖尿病和糖尿病视网膜病变发生发展中起着重要作用。Objective To study the effect of the adiponcetin,TNF-α,sICAM-1 and hs-CRP on the generation and progression of type 2 diabetes mellitus and diabetic retinopathy.Methods The levels of adiponcetin,TNF-α,sICAM-1 and hs-CRP were detected in 35 patients with diabetes mellitus and compared with those in 40 case of type 2 diabetes mellitus without diabetic retinopathy and 50 normal controls.Results The levels of TNF-α,sICAM-1 and hs-CRP in partients with type 2 diabetes mellitus were obviously higher than those in normal controls（P0.01）,the level of adiponcetin was obviously lower than that in normal controls（P0.01）.Compared with those without diabetic retinopatly,the levels of TNF-α and hs-CRP were significantly higher and adiponectin was significantly lower in patients with diabetic retinopathy（P0.01）.The level of serum adiponcetin was negatively related to the levels of TNF-α,sICAM-1 and hs-CRP（r=-0.512,r=-0.517,r=-0.523,P0.05）.Conclusion The adiponcetin has antihyperglycemia and antiinflammatory effects.The decrease of adiponcetin level and the increase of levels of TNF-α,sICAM-1 and hs-CRP may play important roles in the generation and progression of type 2 diabetes mellitus and diabetic retinopathy.
