中药组方与有机硒治疗地方性砷中毒临床效果观察

摘　　要：目的观察中药组方和有机硒对慢性砷中毒病人的治疗效果,探索有效的慢性砷中毒治疗药物。方法选取175例现症病人,男性88人,女性87人,平均年龄51岁,采用2×2析因设计随机(随机数字表)分为4组,即对照组、硒治疗组、中药治疗组、中药加硒治疗组。测量血压和心电图描记,检测病人血清中谷胱甘肽过氧化物酶(GSH-Px)、超氧化物歧化酶(SOD)、谷胱甘肽硫转移酶(GSTs)、丙二醛(MDA)及免疫球蛋白IgG水平,观察治疗前后的变化情况。结果175名病人中共检出高血压者46人,检出率为26.3%;心电图异常检出人数37人,异常率为21.1%。组间以治疗后与治疗前主要指标的差值进行比较:(1)与无中药组比较(不考虑硒),中药组SOD活性高于无中药组(P<0.01),中药组GSTs活性、MDA和IgG水平低于无中药组(P<0.05或P<0.01);(2)与无硒组比较(不考虑中药),加硒组SOD活性高于无硒组(P<0.01),加硒组IgG水平低于无硒组(P<0.01);(3)在对SOD、GSTs活性、MDA和IgG水平影响方面,硒和中药均能提高SOD活性(P<0.01)并呈协同作用(P<0.05);硒能降低MDA、IgG水平(P<0.05或P<0.01);中药能降低GSTs活性、MDA及IgG水平(P<0.05或P<0.01)。结论硒和中药可有效地提高机体清除氧自由基的能力,减轻脂质过氧化的程度,对治疗慢性砷中毒具有一定效果,中药值得在治疗砷中毒方面去研究开发和应用。Objective To observe the clinical effect of endemic arsenism with traditional chinese medication and organic selenium,and to explore the effective medication of treatment of endemic arsenism.Methods One hundred and seventy-five endemic arsenism patients were randomly divided four groups according to factorial experiment design.Four groups included the treatment group with organic selenium,the treatment group with Chinese medication,the treatment group with Chinese medication and organic selenium and the control group.The activities of superoxide dismutase（SOD）and glutathione peroxidase（GSH-Px） and glutathione S-transferase（GSTs）,the levels of malondialdehyde（MDA）and the level of immunoglobulin G（IgG）were determined before and after therapy.Results During 175 patients,the 46 patients was hyperten sion,the detection rate was 26.3%;37 patients was electrocardiogram abnormal,the abnormal rate was 21.1%.The groups were compared in front and behind of treatment：①The four groups were compared by the content medication（without considering selenium）,the activity of SOD in treatment group with traditional chinese medication was higher（P〈0.01）,the activity of GSTs and the levels of MDA and IgG were lower（P〈0.05 or P〈0.01）.② Comparing with the treatment group without selenium（without considering traditional chinese medication）,the activity of SOD in treatment group with organic selenium was higher（P〈0.01）,the level of IgG was lower（P〈0.01）.③The activity of SOD was increased by the joint action of selenium and traditional Chinese medication（P〈0.05）.The levels of MDA and IgG were decreased by the selenium（P〈0.05 or P〈0.01）.The activity of GSTs,the levels of MDA and IgG were decreased by the traditional Chinese medication.Conclusions Selenium and traditional Chinese medication could enhance the antioxidative capability of body and prevent antioxidative damage.Selenium and traditional Chinese medication had the effect on treating for endemic arsenism.Tra
