摘　　要：目的:了解北京市社区美沙酮维持治疗(methadone maintenance treatment,MMT)脱失和保持的影响因素。方法:根据课题设计,于2007年12月-2008年9月,在北京市社区药物维持治疗门诊选择29名服药人员,对每位服药人员单独进行深入访谈,归纳总结服药人员脱失和保持的影响因素。结果:脱失的主要影响因素是:服药人员对美沙酮药物本身或/和MMT普遍存在不正确认知、服药期间偷吸毒品、缺乏必要的社会心理干预、个别服药人员缴费困难、存在服药不够便利的问题和公安部门的配合仍需提高等;有利于保持的因素是:家人支持、给药剂量合适、门诊工作人员的服务态度积极等。结论:加大MMT政策和相关知识的宣传力度,对MMT服药人员积极开展社会心理干预活动,针对北京市美沙酮维持治疗的薄弱环节采取有针对性的防治措施,降低脱失率。Objective：To explore the factors associated with retention or drop out of methadone maintenance treatment （MMT） in Beijing. Methods： According to the study design, a total of 29 MMT patients were recruited from MMT clinics during December 2007 to September 2008. The in - depth or open -ended interview was used to collect information about the factors associated with retention or drop out of MMT. Results：The main reasons of drop out of MMT included： poor knowledge and awareness of methadone and/or MMT, use heroin stealthily during treatment, lack of psycho - social intervention, difficulty to pay treatment charge, inconvenience to reach MMT service, insufficient support from public security departments. Family supports, appropriate methadone dose and good service from MMT clinic staff were the main factors contributed to retention in MMT. Conclusion： A comprehensive measure including further education and propaganda about MMT policy and related knowledge, psycho - social intervention and inter- sectoral coordination between government departments should be strengthened.
