摘 要:目的探讨在心律失常条件下,320排容积CT(VCT)冠状动脉血管成像的图像质量及其临床价值。方法心律不齐14例行320排VCT扫描,以优、良、中、差为标准,评价各支冠状动脉的图像质量分布。以60例心律整齐病例为对照,评价不同心率时,心律不齐对图像质量的影响。结果心率〈70次/min时,心律整齐和心律不齐图像质量分布无统计学差异(P=0.390);心率≥70次/min时,心律整齐和心律不齐图像质量分布有统计学差异(P〈0.001)。心律整齐时,心率过快并不明显影响图像质量(P=0.460);当心律不齐时,心率过快可导致图像质量明显下降(P〈0.001),但图像可诊断率仍保持在96.3%。结论心律失常的情况下,VCTA图像质量仍具有很高的优良率和可评价率,扩大了冠状动脉CTA的应用范围。Objective To evaluate the image quality and clinical value of 320-detector row CT angiography in patients with arrhythmia. Methods 14 cases with arrhythmia underwent 320-detector row CT angiography,according to 4 degrees(excellent,good, middle and bad), the image quality was evaluated and compared with that of 60 cases without arrhythmia. The effect of arrhythmia on image quality was analyzed. Results There was no statistically significant difference between the two subgroups with and without arrhythmia(P = 0. 390) when heart-rate was below 70bpm;while arrhythmia could decrease the image quality significantly when heart -rate was above 70bpm( P〈0. 001). In the subjects without arrhythmia, the heart-rate didn't influence the image quality statistically(P=0. 460). On the other hand, fast beating could result in significant decline of image quality in the group with arrhythmia(P 〈0. 001). However,the rate of images be used in diagnosis still maintained at 96.3 %. Conclusion 320-detector row CT can provide better coronary images even in the patients with arrhythmia ,which expand the applied range of coronary CT angiography.
