摘  要:目的分析女性慢性肾衰竭(chronic renal failure,CRF)患者血液透析前后的嗓音声学特征。方法对28例女性CRF患者血液透析前后的嗓音进行声学分析,选取3秒平稳段持续长元音/a:/声信号作为分析样本,测试其基频(F0)、基频微扰(jitter)、振幅微扰(shi mmer)、最低基频(Flo)、最高基频(Fhi)、嗓音扰动指数(VTI)、软发声指数(SPI)、相对平均扰动(RAP)、噪谐比(NHR)和最大发声时间(MPT)。结果28例CRF患者血液透析后F0、Fhi、Flo及MPT较透析前增加,差异有统计学意义(P<0.05);其余各参数中除VTI外,均较血液透析前增高,但差异无统计学意义(P>0.05)。慢性肾衰患者病程长短与嗓音声学参数无相关性。血液透析前后间接喉镜检查声带无明显改变。结论CRF血液透析后的嗓音声学参数中基频、最高基频、最低基频以及最大发声时间明显增加,与患者主观感觉血液透析后音调提高相一致。Objective This study was conducted with the purpose of analyzing the effects of hemodialysis on the voicing characteristics of patients with chronic renal failure. Methods 28 female patients with chronic renal failure were recruited for the study, ranging in age from 34 to 60 years. Patients underwent evaluation of their voices directly before and after hemodialysis. The vocal acoustic parameters included F0, Flo, Fhi, Jitter, shimmer, NHR, VTI, SPI, RAP and MPT, all measured when voicing a sustained vowel/a.. /. The results were recorded on a software Model 4150(Kay Elemetric Corporation). Results The data were analyzed using the paired t-test for the total sample and showed a statistically significant increase in F0, Fhi ,Flo and MPT after the hemodialyis(p〈0. 05) . Although SPI, RAP, Shimmer and Jitter were noted to have increased, the acoustic findings were insignificant after the hemodialysis. Conclusion The experiment has shown that objective acoustic analysis of voicing parameters can yield equally meaning results as perceptually observed otherwise after the treatment of hemodialysis.
