活体肝移植术前三维超声测量供者肝脏体积与术中肝脏体积和质量的关系研究

摘　　要：目的探讨活体肝移植术前三维超声测量供者右半肝体积与术中测得供肝体积和质量的相关关系。方法术前使用三维超声分别测量供者不含肝中静脉和含肝中静脉的右半肝体积。术后根据采用的术式,使用相应的测量值作为供肝的术前体积(V术前),并将供者分为2组。术中测得供肝的体积(V术中)和质量(W术中)。计算V术前和V术中的偏差,并对V术前与V术中和W术中进行相关与回归分析。结果不含肝中静脉组,V术前与V术中的体积偏差为-3.44%±14.97%,V术前与V术中的相关系数r=0.680,P<0.001,V术前与W术中的相关系数r=0.682,P<0.001;含肝中静脉组,V术前与V术中的体积偏差为6.92%±17.11%,V术前与V术中的相关系数r=0.475,P=0.001,V术前与W术中的相关系数r=0.565,P<0.001。结论三维超声测量供者肝脏体积与术中肝脏体积和质量存在良好的线性相关性。Objective To evaluate the correlation between preoperative volume measured by three-dimensional ultrasonography （3DUS） and its intraoperative volume and weight of graft in living donor liver transplantation （LDLT） . Methods Volumes of right hepatic lobes without and with middle hepatic vein （MHV） of donors were measured by 3DUS respectively before operation. According to the operation manner, we took relevant measured value as the preoperative volume （Vpreop） of graft after operation and divided donors into 2 groups. Intraoperatively the vol- ume （Vintraop） and weight （Wintraop） of graft were achieved. We calculated the deviation of Vpreop from Vintraop and analysed the correlation and regression among Vpreop, Vintraop and Wintraop. Results The group without MHV： the volume deviation was -3.44%±14.97%. Correlation coefficient between Vpreop and Vintraop was 0.680, P〈0.001. Correlation coefficient between Vpreop and Wintraop, was 0.682, P〈0.001. The group with MHV： the volume deviation was 6.92%± 17.11%. Correlation coefficient between Vpreop and Vintraop was 0.475, P=0.001. Correlation coefficient between Vpreop and Wintraop was 0.565, P〈0.001. Conclusions The study shows the 3DUS volumetric measurement has a sig nifieant correlation with the intraoperative volume and weight of graft.
