高频超声诊断跟腱断裂的临床价值

摘　　要：目的总结跟腱断裂的声像图特征,探讨高频超声对本病的诊断价值。方法回顾性总结高频超声诊断的104例跟腱断裂,其中男性81例,女性23例。将术前超声检查结果与手术对比作为金标准,并与35例急性跟腱炎进行比较。根据跟腱断裂程度及声像图特点,将跟腱断裂划分为:完全性断裂、部分性断裂和陈旧性断裂。结果高频超声诊断跟腱断裂的敏感性为98%,漏诊率为2%;特异性为97%,误诊率为3%;与手术的符合率为97.8%。结论高频超声可对跟腱断裂程度进行量化评价,并可明确跟腱断裂的准确位置,为临床治疗提供诊断依据。Objective To summarize the acoustic image feature of achilles tendon ruptures and explore the diagnosic value of high frequency ultrasound for these diseases. Methods 104 cases with ruptures of achilles tendon diagnosed by the high frequency ultrasound were summarized and analysed retrospectively. There were 81 male cases and 23 female cases among these patients. The results of ultrasound examination were compared among the achilles tendon ruptures patients and the acute achilles tendon inflammation patients to use the results in operations as gold standard. The ruptures of achilles tendon were divided into complete ruptures, part ruptures and old ruptures according to the degree of achilles tendon ruptures and the feature of acoustic image. Results The sensitivity of the high frequency ultrasound diagnosis for achilles tendon ruptures was 98%, rate of missed diagnosis was 2%, specificity was 97%, rate of misdiagnosis was 3% and accuracy was 97.8%. Conclusions The high frequency uhrasound may quantitatively evaluate the degree of achilles tendon ruptures and may define the accurate location of achilles tendon ruptures, which can provide accurate diagnosie basis for the clinical treatment.
