摘  要:目的探讨上皮中性粒细胞活化肽78(ENA-78)、干扰素诱导蛋白10(IP-10)和血管内皮生长因子(VEGF)的基因多态性与特发性肺纤维化(IPF)的相关性。方法采用聚合酶链反应-限制片段长度多态性技术,检测60例住院IPF患者(IPF组)和60例年龄、性别、吸烟情况匹配的外伤、骨折住院患者(对照组)的ENA-78(-156G/C)、IP-10(-1596C/T)和VEGF(+405G/C)的基因多态性,以比值比(OR)和95%可信区间(C1)表示相对风险度。结果IPF组ENA-78GC+CC基因型频率(20.0%)和C等位基因频率(12.7%)均明显高于对照组[分别为6.7%(P=0.032)和3.3%(P=0.008)],携带C等位基因的个体患IPF的相对风险度明显增加(OR=4.23,95%CI为1.35~13.20);IP-10CT+CT基因型频率(10.O%)和T等位基因频率(5.8%)均明显低于对照组[分别为26.7%(P=0.018)和14.2%(P=0.031)],携带T等位基因的个体患IPF的相对风险度降低(OR=0.38,95%CI为0.15~0.95)。2组VEGF(+405G/C)基因型和等位基因频率分布差异无统计学意义。结论ENA-78(-156C)等位基因是IPF的危险因素,IP-10(-1596T)等位基因是IPF的保护性因素,VEGF(+405G/C)基因多态性与IPF的发生可能无相关性。Objective To examine whether there was an association between epithelial neutrophil activating peptide 78 ( ENA-78 ), interferon-inducible protein 10 ( IP-10), vascular endothelial growth factor (VEGF) polymorphism and Chinese patients with idiopathic pulmonary fibrosis (IPF). Methods Polymerase chain reaction (PCR) and restriction fragment length polymorphism (RFLP) were performed to analyze the gene polymorphisms of ENA-78 ( -156G/C), IP-10 (-1596C/T) and VEGF (+405G/C) in 60 IPF patients and 60 controls with trauma or bone fracture matched in age, gender and smoking status. Results The distribution of carrying GC + CC genotype frequency (20. 0% ) and C allele frequency (12. 7% ) for ENA-78 in IPF patients was significantly higher than that in healthy controls [6.7% (P = 0. 032 ) and 3.3% (P = 0. 008 ), respectively], the relative risk of suffering from IPF of -156C allele gene carrier significantly increased ( OR = 4.23,95 % CI: 1.35 - 13.20 ). The distribution of carrying CT + TT genotype frequency ( 10.0% ) and T allele frequency ( 5.8% ) for IP-10 in IPF patients was significantly lower than that in healthy controls [ 26.7% ( P = 0. 018 ) and 14.2% ( P = 0. 031 ), respectively], the relative risk of suffering from IPF of - 1596T allele gene carrier decreased (OR = 0.38,95% CI:0. 15 - 0. 95). No association was found between VEGF ( + 405G/C ) polymorphism and IPF. Conclusions - 156C allele for ENA-78 may be a risk factor of IPF and - 1596T allele for IP-10 a beneficial factor of IPF. The VEGF (+405G/C) gene polymorphism has no effect upon the predisposition to IPF.
