左旋卡尼汀对冠心病合并糖尿病心力衰竭30例患者的临床疗效观察

摘　　要：目的观察左旋卡尼汀对冠心病并糖尿病心力衰竭患者心功能及血糖、血脂的影响。方法60例冠心病并糖尿病心力衰竭患者随机分为常规治疗组(对照组,30例)和左旋卡尼汀组(治疗组,30例),对照组给予地高辛、利尿剂、血管紧张素转换酶抑制剂或血管紧张素Ⅱ受体拮抗剂及β受体阻滞剂治疗,治疗组在此基础上加用国产左旋卡尼汀5g静脉滴注,1次/d,连续用药15天。观察治疗前及治疗后患者的心功能及血糖血脂的变化。结果与治疗前相比,治疗组的左心室收缩末容量(LVESV)、左心室射血分数(LVEF)明显改善,同时血糖明显下降,血脂变化不大。结论左旋卡尼汀可以辅助治疗冠心病并糖尿病心力衰竭患者,改善心功能,同时可降低血糖。Objective To evaluate the effects of L - carnitine on the heart function and glucose and lipid metabolism in heart failure patients with coronary heart disease and type - Ⅱ diabetes. Methods sixty heart failure patients with coronary heart disease and type- Ⅱ diabetes were divided into conventional therapy group （control group,30patients） and L- carnitine therapy group （therapy group, 30 patients）. The control group patients were given basic treatment with digoxin,diuretics,angiotensin converting enzyme inhibitor（ ACEI） or angiotensin Ⅱ receptor blocker（ARB） , and β -blocker; the therapy group were given L -carnitine 5gPd for 15 days in addition to the basic treatment. Heart function and glucose＆lipid in heart failure patients was observed before and after L - carnitine treatment. Results After treatment,left ventricular end systolic volume （LVESV） of patients decreased significantly and left ventricular ejection fraction（LVEF） of patients increased significantly in L- carnitine group,accompanied by decrease in blood glucose level. But the changes of blood lipid were not significant. Conclusion L - carnitine can be used as adjuvant therapy to improve the heart function in heart failure patients with coronary heart disease and type - Ⅱ diabetes.
