摘 要:目的:研究扩散加权成像(DWI)在脊髓型颈椎病(CSM)中的应用价值。方法:对26例临床及影像学证实为CSM患者,22例非CSM患者行颈髓MRI和扩散加权成像,分析病变表现并测量其ADC值。结果:48例均获得弥散加权图像和弥散系数。CSM患者受压部位ADC值明显高于邻近部位和正常颈髓ADC值,差异有统计学意义(P<0.05);17例CSM脊髓受压部位T2WI出现高信号,ADC值增高;9例T2WI表现为等信号,其中有6例表现为脊髓受压部位ADC值增高,DWI显示不同信号组受压部位平均ADC值差别无统计学意义(P>0.05),但不同信号组受压部位与相应邻近正常部位平均ADC值比较,差异有统计学意义(P<0.05)。结论:DWI可以通过受压脊髓ADC值改变更早的判断脊髓内部变化,比常规T2WI能更早、更准确显示脊髓受压的情况,从而有助于早期诊断和治疗。Objective:To study the role of diffusion weighted imaging(DWI)in the clinical application of cervical spondylotic myelopathy.Methods:Cervical MRI and DWI were performed in 26 patients with cervical spondylotic myelopathy(CSM) and 22 patients without CSM proved by clinical and radiological findings.The MRI findings were analyzed,and the apparent diffusion coefficient(ADC)value were calculated.Results:ADC could be calculated in all 48 patients.The average ADC value in the region of spinal cord compression was significantly higher than that in adjacent region in CSM patients as well as that in normal cervical cord(P〈0.05).There were 17 cases showed hyperintensity on T2WI and increased ADC value in the region of spinal cord compression in CSM patients.Normal T2WI was shown in 9 cases.Among these,six cases had increased ADC value at the region of spinal cord compression.There were significantly different of the average ADC value between the region of spinal cord compression and the adjacent region in two different signal groups(P〈0.05).Conclusion:DWI can be used to find spinal cord interior change earlier,with higher sensitivity than that of conventional MRI,which might be helpful for early diagnosis and treatment of cervical spondylotic myelopathy.
