摘  要:背景:激光可用于高效外科手术止血,其在外伤止血方面也具有广泛的应用前景。目的:通过测定止血时间和3种反映凝血机制指标的变化情况以及进行组织化学染色,对532nm激光局部照射的止血作用进行考察。材料和方法:10只成年雌性健康的新西兰大耳白兔,体重(2.5~3.0)kg。10只家兔左右耳进行对照实验。在耳背距耳尖5cm处中央作(0.6~0.8)cm长,深及中央动静脉的横切口(左右两耳被切血管粗细相近)。左耳传统组,用传统的压迫伤口止血法止血;右耳为激光照射组,用5W 532nm激光照射伤口后再按压止血。用秒表测量止血时间(s)。止血后取心脏处血液测试凝血酶原时间(PT),活化部分凝血活酶时间(APTT)和血小板(PLT)数量。伤后第3天、第7天、第10天、第20天和第30天对伤口取活检,做常规苏木精-伊红染色,在普通光学显微镜下观察组织形态变化。结果:10只家兔均进入结果分析。①与传统组对照,激光照射组伤口止血速度快,差异具有显著性意义(P<0.05)。②与传统组对照,激光照射组PT时间增长,APTT时间缩短,PLT数量增加,三种指标差异有统计学意义(P<0.05)。③与传统组对照激光照射组组织损伤加重,伤口愈合时间更长。Background: Laser shows good hemostatic effect on surgical operation, which also illustrates a good prospect of application in the field of wound hemostasis. Objective: To observe the hemostatic effect on local irradiation wound by 532nm la ser, hemostatic time was tested, the changes of three indicators for blood coagulation were measured and the histoehemical stain study was conducted. Materials and Methods: Ten healthy, grown, female New Zealand Rabbits weighing (2.5-3.0) kg were selected. Control experiment was carried out with 10 rabbit. A wound of cutting center artery and vena was made on ever y ear back of each rabbit, which was (0.6-0.8) cm in length, 5cm in distance from ear tip. The left ear group was named as tradition group, which was stanched by traditional pressure methods. The right ear group was named as Laser irradiation group, which was irradiated by 5W 532nm laser and then pressed to stanch. Hemostatic time was tested. Hemoglutination a- nalysis experiment was performed with heart blood, Prothrombin time (PT), Activated Partial Thromboplastin time (APTT) and platelet (PLT) number were measure. The wounds were biopsied separately and specimens were observed after HE staining under microscope after 3d, 7d, 10d, 20d and 30d. Results: All 10.rabbits were included in result analysis. ①Hemostatic time of laser irradiation group was significantly shorter than tradition group(P〈0.05). ② Contrasted with tradition group, PT of laser irradiation group was longer(P〈0.05), APTT of laser irradiation group was shorter(P〈0.05), PLT number of laser irradia tion group was more(P〈0.05).③ Tissue damage got worse, and the time of tissue healing got longer compared to tradition group.
