HLA-DR抗原在不同中医证候幽门螺杆菌感染胃痛患者胃上皮中的表达

摘　　要：目的:探讨胃痛患者幽门螺杆菌(Hp)感染、胃上皮细胞HLA-DR抗原表达以及中医证候之间的关系。方法:对57例脾胃湿热证、24例肝气犯胃证和19例脾胃虚寒证患者,采用免疫组化方法分析胃窦上皮细胞HLA-DR抗原的表达,用改良Giemsa染色法确定Hp感染程度。结果:HLA-DR抗原表达在Hp-组与Hp+组中差异显著(P=0.00;P<0.01);脾胃湿热和肝气犯胃证组Hp定植密度与HLA-DR抗原表达呈正相关性(r=0.33,P=0.01;r=0.45,P=0.03;P均<0.05),但脾胃虚寒证组两者未有统计学意义的相关性(r=0.46,P=0.05)。Hp定植密度和HLA-DR抗原的表达在3种中医证候组无显著性差异。结论:Hp感染增加了HLA-DR抗原的表达,Hp感染程度和HLA-DR抗原表达与中医证候的形成相关性不大。Objective： To study the relationship among density of Helicobacter pylori（ H .pylori）, expression of HLA- DR antigen in gastric epithelial cells and development of different TCM syndromes of stomach-ache. Methods： Gastric mucosal biopsy specimens from 57 cases of spleen-stomach dampness-heat syndrome, 24 cases of liver-qi attacking stomach syndrome and 19 cases of spleen-stomach deficiency -cold syndrome were analyzed. The expression of HLA-DR antigen was determined by immunohistochemistry. H.pylori colonizing density was determined by modified Giemsa stain. Results： The expression of HLA-DR antigen was significant difference in H.pylori＋ group vs H.pylori- group （ P=0.00; P〈0.01 ） . There was correlation between H.pylori colonizing density and HLA-DR antigen expression in spleen-stomach dampness-heat syndrome and in liver-qi attacking stomach syndrome （ r=0.33, P=0.01 ; r=-0.45, P=0.03 ; P〈0.05 ） , but no significant correlation in spleen-stomach deficiency -cold syndrome （ r=-0.46, P=0.05 ） . Among the three TCM syndromes, neither H.pylori colonizing density nor HLA- DR antigen expression was significant difference. Conclusion： H.pylori infection increases the extensive expression of HLA-DR antigen in the gastric epithelia. There was no relationship between the colonizing density of H.pylori and the development of TCM syndromes. The level of HLA-DR antigen expression has no correltionship
