妊娠合并糖代谢异常孕妇性激素结合球蛋白(SHBG)水平与母儿合并症的关系

摘　　要：目的:研究妊娠早期正常孕妇、妊娠期糖尿病(GDM)及妊娠期糖耐量异常(GIGT)孕妇性激素结合球蛋白(SHBG)水平与母儿合并症的关系。方法:对正常妊娠组420例和妊娠期糖代谢异常组304例进行50g葡萄糖负荷实验(GCT)检测,其中GCT结果异常者再进行75g葡萄糖耐量实验(OGTT);同时对所有研究对象均进行SHBG的测定并根据其水平高低分组进行母儿合并症的统计。结果:早孕期妊娠合并糖代谢异常孕妇两组血清SHBG值均较正常孕妇者低(P<0.001),而妊娠期糖代谢异常患者的母儿并发症随SHBG水平的降低而增多。结论:SHBG在妊娠期糖代谢异常的早期筛查诊断及母儿预后评估中具有重要意义。Objective：To investigate the relationship between maternal and fetal complications and the levels of sex hormone-binding globulin（SHBG）in normal pregnant women,gestational diabetes mellitus（GDM） and gestational impaired glucose tolerance（GIGT） pregnant women during early pregnancy.Methods：50g glucose challenge test（GCT） was checked for 420 cases of normal pregnant women and 304 cases of pregnant women with abnormal glucose metabolism,75g oral glucose tolerance test（OGTT） was checked for the pregnant women whose GCT results were abnormal.At the same time,all subjects were carried out determination of SHBG and grouped according to their incidence of maternal and fetal complications.Results：During early pregnancy,serum SHBG values of the pregnant women with abnormal glucose metabolism were lower than those of normal pregnant women（P〈0.001）,and the incidence of complication was increasing with the decline of SHBG in the study group.Conclusion：The determination of SHBG is important in screening,diagnosing and assessing the prognoses of the gestational abnormal glucose metabolism and infants.
