聚氨酯铋输卵管栓堵术18～24个序月的临床研究

摘　　要：为进一步了解聚氨酯铋输卵管栓堵的效果,采用一次性医用带囊双腔管,经宫腔注入聚氯酯铋行输卵管栓堵术585例,观察18～24个序月的栓堵效果及术后X光片显影,对10例术后检查输卵管通畅者实行补做。结果,随访至18个月者409例(70％);24个月者176例(30％),11例妊娠均发生在18个月内。术后2～3个月行输卵管通畅试验,其中造影者132例,123例堵塞成功;通水试验442例,成功383例。总成功率为86.7％,累积妊娠率为1.8％,无术中术后反应。经宫腔注入高分子材料的输卵管栓堵术为一种简便价廉、无副反应、不经腹部的输卵管绝育术。术后X光片检查双显者可做为成功指标,术后不能肯定者,需做输卵管通畅试验。不成功者立即补做,可提高成功率。Objects: the survey of a new nonsurgical tubal sterilization by injecting a noncorrosive polymolecular substance - bismuth polyurathan by the way of uterine cavity.Methods: Through a specific double channelled polyethylene tube with a rubber bulb, bismuth polyurathan was injected into the uterine cavity, then into the fallopian tubes forming thrombi there as a nonsurgical method of tubal sterilization. From Nor. 1993 to April. 1994, 585 women had received this sort of operation and followed up for 18 to 24 montths was 409 (70%) while that for 24 months, 176 (30%), A pelvic X- ray film was taken immedietely after operation and evaluated as double, Single, non visualized and doubtful. Besides 11 pregnancies discovered within 18 months after operation. The patency of tubes was examined by HSG in 132 cases with obstruction in 123 cases, by hydrointubation in 442 cases with obstruction in 383 cases, Ten failures received the second attempt.Results: Eleven caes resulted in pregnancy making a cumulative pregnancy rate of 1.8%, The total failure rate with the addition of patent trbes was 13.3%, i, e, the successful sterilization rate was 86.7 % , No side reactionsissued. Both tubes visualized postoperatively reflected successful tubal obstruction in 97.2% , so it could be used as an index for operative success. Ten second attempts all resulted successfully.Conclusion: this is a very hopeful new nonsurgical tubal sterilization method. The specific double channelled tube is much better than the reformed Foley catheter reported in our last report of 259 cases.
