中西医结合治疗肾综合征出血热急性肾功能衰竭疗效及血压、脉搏和尿量积分值的观察

摘　　要：目的：观察中西医结合抢救肾综合征出血热（ＨＦＲＳ）重型急性肾功能衰竭（肾衰）的疗效，并用自行设计的血压、脉搏、尿量积分值评价疗效的临床意义。方法：采用十枣汤为主中西医结合抢救ＨＦＲＳ重型急性肾衰３１例（治疗组），并与西医治疗３０例（对照组）对照，比较２组治疗前后的血压、脉搏和尿量总积分值。结果：治疗组痊愈率为９０．３％（２８／３１），总有效率为９６．８％（３０／３１），病死率为３．２％（１／３１）；对照组分别为６６．７％（２０／３０）、７３．３％（２２／３０）和１０．０％（３／３０），２组间均有显著性差异（Ｐ均＜０．０１）。治疗组治疗前血压、脉搏及尿量总积分值为（７９．７±８．９）分，治疗后降为（２７．１±５．２）分（Ｐ＜０．０１），且治疗组下降程度明显优于对照组（Ｐ＜０．０１）。结论：以十枣汤为主的中西医结合方法抢救ＨＦＲＳ急性肾衰疗效满意；本记分法具有准确体现病情。Objective:To observe the therapeutic efficacy of TCM WM in saving severe acute renal failure (ARF) induced by hemorrhagic fever renal syndrome (HFRS) and to evaluate its clinical significance through score value of blood pressure,pulse and urine volume using selfdesign method. Methods :Thirtyone cases of severe ARF due to HFRS were treated with TCM WM with Shizao Decoction in chief (treatment group) and compared to 30 cases treated with western medicine (control group).Then,to observe the scores of blood pressure,pulse and urine volume before and after therapy.Results:In treatment group,cured rate was 90 3% (28/31),total effective rate was 96 8% (30/31) and death rate was 3 2%(1/31);but in control group,cured rate was 66 7%(20/30),total effective rate was 73 3%(22/30) and death rate was 10 0% (3/30),there had obvious difference between 2 groups (all P <0 01).Moreover, in treatment group the total scores of blood pressure,pulse and urine volume before and after treatment were (79 7±8 9) scores and (27 1±5 2) scores respectively,the decrease degree in treatment group was superior to that in control group ( P <0 01).Conclusions:There have a satisfactory therapeutic efficacy of TCM WM with Shizao Decoction in chief saving ARF due to HFRS;this score method has a clinical significance in understanding disease,evaluating efficiency and judging prognosis.
