厄贝沙坦联合金水宝胶囊治疗老年2型糖尿病肾病的临床疗效观察

摘　　要：目的观察厄贝沙坦和金水宝胶囊联合治疗老年糖尿病肾病的临床疗效。方法将60例老年糖尿病肾病患者随机分为厄贝沙坦75mg组（A组）、厄贝沙坦150mg组（B组）及厄贝沙坦150mg联合金水宝胶囊组（C组），每组20例。比较各组治疗前后24小时尿蛋白定量（UTP／24h）、血肌酐（Scr）、尿素氮（BUN）、血脂（Lipids）、N-乙酰-B—D葡萄糖苷酶（NAG）、收缩压（SBP）、舒张压（DBP）的变化。结果3组患者的血压在治疗后均有下降（P〈0．05），但组间比较无显著性差异（P〉0．05）；3组患者治疗后UTP／24h均有下降（P〈0．05），C组下降最为明显，与A组比较差异有显著性（P〈0．05）；A组和B组治疗后Scr、BUN、NAG无明显改变（P〉0．05），与A组和B组比较，C组在治疗后Scr、BUN、NAG下降明显（P〈0．05）；与治疗前比较，A、B组治疗后TG、TC、LDL-C、HDL—C均无明显变化（P〉0．05），C组治疗后TG、TC、LDL-C下降，HDL-C升高（P〈0．05）。结论厄贝沙坦联合金水宝胶囊治疗糖尿病肾病能有效降低糖尿病肾病患者蛋白尿，显著改善肾功能。Objective To Observe the Chnical therapeutic effect of Jinshuibao and Irbesartan on patients with Diabetic Kidney Disease（DKD）. Methods 60 cases of patients with DKD were given basic treatments （ including control of blood glucose, blood lipid, dietary restrictions ）, then all the patients were randomly divided into group（A） with 75 mg Irbcsartan,group（B） with 150 mg Irbesartan and group（C） with 150 mg Irbesartan plus Jinshuibao. UTP/24 h,Scr,BUN、NAG,SBP,DBP、Lipids were measured respectively before and after 75 mg Irbesartan, 150 mg Irbesartan,or Irbesartan 150mg plus Jinshuibao treatment. Results Compared with group A, UTP/24 h was decreased remarkably in group B and group C （P 〈0.05） ;Ser、BUN,NAG,TG、TC and LDL-C were significantly lower in group C compared with group A and group B （P 〈 0.05 ） ;But HDL-C was much higher in group C than group A and group B （P 〈 0.05 ）. Conclusion Combined treatment of Irbesartan and Jinshuibao to patients with DKD could reduce the proteinuria and improve renal function significantly.
