摘 要:目的探讨术前肠内免疫营养联合益生菌对结直肠恶性肿瘤患者术前肠道准备临床应用效果。方法将同意进行实验74例的结直肠癌患者,随机分为常规治疗组(CONT组)、普通肠内免疫营养组(NEN组)、添加益生菌肠内免疫营养组(EEN组)。观察患者术中肠道清洁度、术后排气时间、术后切口感染情况以及肠道菌群失调情况,并监测手术前后血浆蛋白、肝功能、淋巴细胞计数、免疫球蛋白改变。结果术后排气时间EEN组及NEN组均早于CONT组(P<0.05),而EEN组早于NEN组(P<0.05);第1次排便后细菌计数结果示EEN组及NEN组均明显低于CONT组(P<0.05),而EEN组低于NEN组(P<0.05)。三组营养后EEN组和NEN组较CONT组总蛋白和白蛋白水平显著升高(P<0.05),营养后EEN组较NEN组和CONT组T细胞水平、IgA水平显著升高,谷草转氨酶、感染率显著性降低,NEN组和CONT组相比差异无显著意义。结论对于结直肠恶性肿瘤患者,术前应用含益生菌的肠内免疫营养支持方法可以有效防治患者肠道菌群失调,促进肠道功能恢复,改善机体的营养状况,提高免疫功能,降低炎症反应,减少术后感染发生率并促进肝功能的恢复。Objective To study the clinical effects of the pre-operative enteral nutrition combined with probiotics for colorectal cancer patient pre-operation bowel preparation. Method 74 colorectal cancer patients were voluntarily selected, and were randomly divided into enteral nutrition immune group(EEN group), enteral nutrition general immunity group(NEN group)and conventional treatment group(CONT group ). Liver function, lymphocyte count, NK cell count, immune globulin, C-reactive protein before and after the operation was observed. The intestinal function recovery time, the incidence of post-operative complications before and after nutritional support was observed. Result For the intestinal function recovery time, group EEN and group NEN are shorter than eont group (P 〈0.05), especially the time in group EEN is shorter than that of group NEN(P〈0.05) After the first Defecation, bacteria count of group EEN and group NEN are less than cont group (P〈0.05). and the number of the EEN group is less than ENE group ( P〈0. 05). The lever of total protein and albumin of EEN group and NEN group are higher than that of CONT group (P〈0.05). After the nutrition, the level of T ceils and IgA of the EEN group and EEN group are significantly increased than that of NEN group and CONT group, and the lever of the AST and the infection rates of the EEN group are decreased greatly, but there is no significant difference between the NEN group and CONT group. Conclusion For colorectal cancer patients, preoperative application of probiotics combined with the intestinal nutrition immune method could improve the patients' nutritional status and enhance immune function, reduce inflammation and the incidence of postoperative in^ection, and to promote the restoration of liver function.
