内镜下鼻结构异常的微创整形治疗接触性头痛

摘　　要：目的:评估内镜下鼻结构异常微创整形治疗鼻内接触性头痛的疗效。方法:对25例鼻内接触性头痛患者行内镜下微创整形,包括中鼻甲成形、窦口鼻道复合体功能性切除、局限性鼻中隔黏膜下矫正。手术结合内镜检查和鼻窦CT评估,将上述各个单一手术进行组合,制定个性化微创整形方案。结果:术后随访9～12个月,平均10个月。痊愈21例(84.00%),有效3例(12.00%),无效1例(4.00%),总有效率96.00%。结论:鼻内接触性头痛是由于鼻腔存在多种结构异常而引起黏膜接触的一类鼻源性头痛。鼻内镜下对双侧鼻腔进行统一的结构、功能、形态的微创整形,恢复鼻腔对称性整体结构可有效治疗接触性头痛。Objective To discuss the effect of endoscopic surgical treatment and its effect on. intranasal contact point headaches. Methods Twenty five patients with intranasal contact point headaches were treated by endoscopic rhinoplasty of in nasal cavity, included middle turbinoplsaty,functional resection of ostiomeatal complex, and endoscopic submucous septoplasty. Made organic combination of the above surgery methods according to different results of the CT scan and endoscopy of each patient, the surgical procedure were designed individually. Results All patients were followed up for 9 to 12 months, average 10 months. 21（84.00%） patients recovered, 3（12.00%） was effective, and 1（4.00%） was in failure. Total effective rate was 96.00%. Conclusion Intranasal contact point headache is a nasal headache of a sort, as a resuh of multi- abnormality of nasal cavity structure. Endoscopic rhinoplasty is an effective treatment by means of reconstructing the balance of bilateral nasal cavity and improving its function.
