压疮预报追踪记录表在老年骨折患者中的应用

摘　　要：目的探讨压疮预报追踪记录表在降低老年骨折患者的压疮发生率的应用方法和效果。方法自2007年开始对≥65岁的老年骨折患者进行压疮风险评估。对分值≤14分的患者进行高危压疮预报,制定预报措施,追踪记录,并进行三级护理质量监控。结果提高了护士的责任心和预防压疮的意识,有效降低老年骨折患者的发生率。结论压疮评估联合预防措施追踪反馈可最大限度降低压疮的发生。Objective To investigate the approach and effect of the stamped sore forecasted tracing descriptive chart in lowering the incidence of stamped sore of old age bone fracture patients. Method Since 2007, we initiate to evaluate the risk of stamped sore in bone fracture patients of old age whose age exceed 65, meanwhile to carry out prediction of high risk stamped sore, to formulate precaution, tracing record and to monitor three-stage quality of care. Result Stamped sore forecasted tracing descriptive chart can raise the nurse＇ s responsibility and consciousness of stamped sore precaution, effectively lower the incidence of stamped sore of old age bone fracture patients. Conclusion The evaluation of stamped sore in association with the tracing feed back of precaution can utmost lower the incidence of stamped sore.
