摘  要:目的评价智能化高胰岛素—正常葡萄糖钳夹(简称葡萄糖钳夹,IGC)技术的临床实用价值。方法应用IGC技术对20例实验对象[正常糖耐量(NGT)16例,糖耐量减低(IGT)4例,其中合并多囊卵巢综合征(PCOS)2例]进行试验,3例NGT者在1月内重复试验。结果 (1)23例次钳夹试验进入稳态期时间为105.57±26.72min;(2)稳态期血糖离散值为0.10±0.06mmol/L;血清胰岛素水平94.64±13.72pmol/L;(3)3例NGT者重复试验稳态葡萄糖利用率(GUR)差值为-0.19±0.91mg·kg^(-1)·min^(-1)(P=0.754);(4)IGC测定的GUR与HOMA1-IR的线性相关系数为R=0.614(P=0.011),与HO-MA2-IR线性相关系数为R=0.593(P=0.02)。结论 IGC技术操作相对简捷,较快进入稳态,血糖钳夹精确性高,稳定性和重复性好。HOMA-IR指数与钳夹技术测定的GUR有显著的线性关系。Objective To evaluate the intelligent hyperinsulinemic euglycemic clamp technique for the clinical practicality and reproducibility. Methods The intelligent glucose clamp(IGC) technique was used in 20 subjects, including 16 subjects with normal glucose tolerance, 4 subjects with IGT, 2 subjects with polycystic ovary syndrome(PCOS). In a month,the IGC test was repeated in three subjects of normal glucose tolerance. Results (1)The average of steady-state experimental time were 105. 57±26. 72 minutes. (2)In steady-state, the average discrete value of blood glucose was 0. 10±0. 06mmol/L, the average of serum insulin levels was 94.64±13. 72pmol/L. (3)The average margin of glucose utilization(GUR)in the repeated test of 3 normal individuals was -0. 19±0. 91mg/(kg·min) (t = 0. 385, P=0. 754) (4) In euglycemic clamp test, the linear correlation coefficient between GUR and HOMAI-IR was 0. 614(P= 0. 011), and linear correlation coefficient between GUR and HOMA2,-IR was 0. 593(P=0. 02). Conclusions Intelligent glucose clamp test is easy and rapid for stepping into steady-state, and it has high accuracy, stability and reproducibility. There is significant linear relationship between HOMA-IR index and GUR tested by clamp technique.
