摘  要:目的研究2型糖尿病患者ATI受体基因1166A/C(AT1R 1166A/C)与醛固酮合酶(CYP11B2)基因—344C/T(CYP11B2—344C/T)多态性与糖尿病肾病(DN)的相关性。方法研究对象分为DN组73例,糖尿病非肾病(non-DN)组72例及正常对照(NC)组52名。应用聚合酶链反应-限制性内切酶片断长度多态性(PCR-RFLP)方法检测各组基因多态性。结果 DN组AT1R 1166A/C AC基因型和C等位基因频率高于Non-DN组(P均<0.05)。DN组AT1R 1166A/C各基因型临床指标比较,AC基因型患者的BUN、Cr、UA、UAER、SBP和DBP均明显高于AA基因型患者。2型糖尿病患者AT1R 1166A/C多态性与DN相关(P<0.05),但CYP11B2—344C/T多态性并未发现与DN相关。结论蚌埠地区汉族人群存在AT1R 1166A/C多态性,其AC基因型与糖尿病肾病有显著相关性。Objective To explore the association of polymorphism of angiotensinⅡ type 1 receptor (AT1R) gene 1166A/C and aldosterone synthase(CYP11B2) gene-344C/T with type 2 diabetes nephropathy development. Methods DN group,non-DN group and normal control were included. PCR- RFLP methods were used. Results The frequencies of the AC genotype and C allele of AT1R 1166A/C in DN were higher than those in non-DN (all P〈0.05). The frequencies of T allele of CYP11B2--344T/ C in DN were higher than those in non-DN or NC (P〉0.05). The clinical indexes of Cr,BUN,uric acid (UA),UAER,SBP,DBP of AC genotype of AT1R 1166A/C were significantly higher than those of AA genotype in DN. The polymorphism of AT1R 1166A/C was correlated with diabetic nephropathy in type 2 diabetes mellitus(P〈0. 05), however the correlation with the polymorphism of CYP11B2- 344C/T was not found in DN(P〉0. 05). Conchtsions The polymorphism of AT1R 1 166A/C exists in Han nationality of Bengbu city and AC genotype is significantly correlated with diabetic nephropthy in type 2 diabetes.
