摘　　要：目的:分析我国中小城市社区卫生服务(community health service,CHS)体系建设的现状和问题,提出相应的发展对策。方法:通过现场考察、问卷调查等方法收集资料,采用主题框架分析和SWOT分析方法探讨中小城市CHS建设的现状、问题和对策。结果:我国中小城市CHS建设尚处于起步阶段,经费和人才问题是当前面临的主要困难。该类城市CHS建设特点为:(1)易于推行与管理;(2)公共卫生职能更加突出;(3)社区卫生服务站优势明显;(4)二级医院具有方便、快捷、可及等社区服务特点;(5)多数城市政府财力较为薄弱;(6)卫生资源总量并不缺乏。结论:建议在中小城市CHS建设中注重加强领导层开发,根据城市特点调整功能定位,合理利用资金,并积极与大城市开展技术合作,同时加强服务质量管理。Objectives： This article aims to analyze the problems of the community health service （CHS） system construction in medium and small cities, and to propose the countermeasures for the CHS construction in this kind of cities. Methods： Through the methods of field survey, expert consultation and questionnaire investigation, statistical analysis, the-matic framework analysis and SWOT analysis are carried out to discuss the situation of CHS system construction in medium and small cities. Results： The CHS system construction in the medium and small cities is still at the initial stage, with different paces. The financial and human resources are the major constraints that the medium and small cities are confronted with. The medium and small cities have the following characteristics：（1）Due to small scale and low population density, the community health service is easier to be implemented and managed. （2）The function of public health is more prominent. （3）CHS stations have obvious advantages.（4）The secondary hospitals in this kind of cities are more accessible with the characteristics of convenient, quickly, as well as CHS. （5）The governments in most of this kind of cities are weak on the fiscal capacity, so the financial input into the CHS system construction is insufficiently.（6）The total health resources in the cities are not defi- cient, and they should be rationally utilized. Conclusion： We should pay more attention to strengthen the development of leadership, adjust the functions of CHS, build the technical cooperation with the larger cities, strengthen the service quality management, and make rational use of funds so as to promote the progress of CHS system construction in medium and small cities.
