重组链激酶对前壁及广泛前壁心肌梗死患者溶栓前后心脏功能的影响

摘　　要：目的通过容量负荷试验评价重组链激酶对急性心肌梗死（AMI）患者溶栓前、后心脏功能的影响。方法选择17例急性前壁及广泛前壁AMI患者进行重组链激酶溶栓，溶栓前、后24h内行漂浮导管连续监测，进行溶栓前后的比较。结果与溶栓前比较，重组链激酶溶栓后6h心功能明显改善，平均肺动脉压由（3．87±0．61）kPa下降至（3．61±0．59）kPa（P〈0．01），肺嵌压由（2．81±0．34）kPa下降至（2．67±0．42）kPa（P〈0．01），心脏指数由2．52±0．49升高至2．72±0．47（P〈0．01）。结论应用重组链激酶溶栓可明显改善AMI患者的心功能。Objective To assess the effect of the recombinant streptokinase （r-sk） thrombolytic therapy on cardiac function in patients with AMI. Methods Hemodynamic monitoring was performed immediately at the time of hospital admission and 24 hours after the r-sk thrombolytic treatment on seventeen patients with anterior wall AMI. Results At 6 hours after the r-sk thrombolytic therapy MPAP decreased from （3.87±0.61）kPa to （3.61±0.59）kPa （P〈0.01） and PCWP decreased from （2.81±0.34）kPa to （2.67±0.42）kPa（P〈0.01）, CI enhanced from 2.52±0.49 to 2.72±0.47 （P〈0.01）. Conclusion The date shows the recombinant streptokinase （r-sk） throm-bolytic therapy significantly improves cardiac function.
