妊娠晚期妇女全血细胞参数与凝血指标检测分析

摘　　要：目的探讨妊娠晚期妇女全血细胞参数以及凝血4项指标的变化及意义。方法采用日本Sys-mex XE 2100 5分类血细胞分析仪S、ysmex CA6000全自动凝血仪,对180例妊娠晚期妇女和110例健康育龄期妇女进行全血细胞参数以及凝血4项指标的检测。结果妊娠晚期组的白细胞计数、中性粒细胞百分比、中性粒细胞绝对值、红细胞体积分布宽度,均比正常对照组增高;而淋巴细胞百分比、淋巴细胞绝对值、红细胞、血细胞比容、血红蛋白、平均红细胞体积、平均红细胞血红蛋白量、平均红细胞血红蛋白浓度、血小板平均体积及大血小板比率,均比正常对照组减低,两者之间均有显著性差异(P均<0.05)。血小板、血小板平均体积以及血小板体积分布宽度,在两组之间则无显著性差异(P>0.05)。与正常对照组比较,妊娠晚期组的血浆凝血酶原时间、活化部分凝血活酶时间均缩短,而纤维蛋白原含量增高、凝血酶时间延长,两者之间均有显著性差异(P均<0.05)。结论妊娠晚期妇女多伴有不同程度的贫血,且以小细胞低色素性贫血多见;白细胞明显增多以中性粒细胞增多为主。妊娠晚期妇女血液呈高凝状态,容易引起妊娠高血压综合征以及弥散性血管内凝血。Objective To explore the changes and clinical significance of complete blood cell parameters and blood coag- ulation indices in parturient women. Methods 180 cases of parturient women （parturient group） and 110 cases of non -pregnant women （control group） were detected for blood cell parameters and blood coagulation indices with Sysmex XE 2100 automatic blood cell analyzer and Sysmex CA6000 automatic blood coagulation analyzer. Results In parturient group,WBC, NEUTr, NEUT, RDW- CV were showed a significant increase in comparison with control group, while LYMPHr, LYMPH, RBC, HCT, HGB, MC~, MCH, MCHC, MPV, P - LCR were decreased significantly （ P 〈 O. 05）. There were no significantly differences in PLT, PCT and PDW（ P 〉 0,05）. Compare with control group, PT and APTT of parturient were shorter, but FIB and TF were increased. There was significant difference in two groups（ P 〈 O. 05）. Conclusion Parturient women are with varying degrees of anemia,and microcytic hypochmmic anemia should be the major type. NEUT could increase mainly in WBC - increasing. Hypercoagulabale state could occur in parturient women, which were liable to hypertension syndrome and disseminated intravascular coagulation （DIC）.
