摘　　要：目的应用动态血压监测评价替米沙坦-氢氯噻嗪治疗轻、中度原发性高血压的长效降压作用。方法开放性临床试验设计,安慰剂导入期2wk筛选轻、中度原发性高血压病人24例,接受替米沙坦治疗2wk(80mg,po,qd),换成替米沙坦-氢氯噻嗪(80mg/12.5mg,po,qd),持续6wk。在安慰剂导入期末和活性药物治疗期末分别行24h动态血压监测。结果经过8wk治疗后,所有病人24h平均收缩压(SBP)和舒张压(DBP)的谷峰比值分别为0.78和0.61,24hSBP和DBP的平滑指数分别为1.8±s0.8和1.6±0.6,24hSBP和DBP分别较安慰剂导入期末降低(27±11)和(16±6)mmHg,日间与夜间SBP及DBP降低幅度相近,不良反应为轻到中度。结论替米沙坦-氢氯噻嗪治疗轻、中度高血压降压效果持久且平稳。AIM To evaluate the long-lasting effect of telmisartan/hydrochlorothiazide by 24 ambulatory blood pressure monitoring in patents with mild to moderate primary hypertension. MATHODS This study was designed as an open label study. After 2 wk of run-in period, 24 patients with mild to moderate primary hypertension were included to receive telmisartan （80 mg, po, qd） for 2 wk, following by telmisartan/ hydrochlorothiazide （80 mg/12.5 mg, po, qd） for 6 wk. Twenty-four hours＇ ambulatory blood pressure was monitored before and after the treatment. RESULTS After 8 weeks＇ treatment, the mean trough/peak （T/P） ratios for systolic blood pressure （SBP） and diastolic blood pressure （DBP） were 0.78 and 0.61, respectively. The mean smoothness index was 1.8 ± s 0.8 for SBP and 1.6± 0.6 for DBP, respectively. The reductions of 24 h SBP and DBP from baseline were （27 ±11 ） and （16± 6） mmHg, respectively. The extents of reduction in both day and night SBP and DBP were comparable. Only mild to moderate adverse reactions were observed during the study. CONCLUSION Telmisartan 80 mg/hydrochlorothiazide 12.5 mg once daily fixed dose combination can provide a lasting and stable antihypertensive effect for treating Chinese patients with mild to moderate hypertension.
