摘 要:目的:研究端侧吻合技术治疗臂丛神经损伤的可行性,为其临床应用提供实验依据。方法:Wistar大鼠60只,随机分为3组,每组20只,每只右侧神经外膜开窗(0.12cm×0.12cm),将臂丛后束损伤后的桡神经远断端端侧吻合于外膜开窗的内侧束远端0.15cm部位,左侧不做吻合。分别于术后4、8、12周在麻醉下行电生理、肌纤维截面积、肌肉湿重及运动终板面积的检测。结果:桡神经端侧吻合后,随时间的延长,运动神经诱发电位潜伏期缩短,波幅增高,传导速度加快,肱三头肌肌湿重及肌纤维截面积值逐渐加大;实验侧与对照侧的肱三头肌肌湿重、肌纤维截面积、运动终板的面积比较,具有非常显著性差异(P<0.01)。结论:端侧吻合能促进臂丛损伤神经再生,并能保护靶肌肉及其运动终板。Objective: To study the effect of end-to-side neurorrhaphy on treatment of brachial plexus injuries in rats, in order to provide experimental evidence for its clinical application. Methods: 60 Wistar rats were randomly divided into three groups, with 20 rats each. The right radial nerve was repaired with end-to-side fashion after windowed, the distal end being sutured to the epineurial window (0.12 cm×0.12 cm) 0.15 cm distal of the medial fasciculus. The left radial nerve was only windowed as control. Electrophysidogical, cross sectional area of the muscle fibers, muscle wet weight and the moter endplate area were detected 4, 8, 12 weeks postoperation respectively. Results: After the end-to-side neurorrhaphy, in the test side, the latency of evoked potential in motor nerve decreased while amplitude and nerve conduction velocity increased. The cross sectional area of the muscle fibers and muscle wet weight also increased gradually. Significant difference was seen between the two groups in the muscle wet weight, the muscle fiber cross sectional area and the moter endplate area (P〈0.01). Conclusion: End-to-side neurorrhaphy can promote nerve regeneration of brachial plexus after injury, also protect target muscle and the moter endplate.
