摘 要:目的探讨银杏叶提取物对绝经后女性冠心病患者冠状动脉血流、血浆一氧化氮(NO)和内皮素(ET)水平的影响。方法将60例经冠状动脉造影证实的绝经后女性冠心病患者随机分为银杏叶提取物治疗组和对照组(各30例),两组均于治疗前、后采用经胸多普勒超声心动图检测绝经后女性冠心病患者冠状动脉左前降支(LAD)血流,并采用硝酸还原酶法和放免法测定血浆NO和ET水平的变化。结果银杏叶提取物治疗可显著增加LAD舒张期峰值流速(P<0.01),收缩期峰值流速(P<0.01),舒张期时间速度积分(P<0.01);同时,血浆NO水平升高(P<0.01),ET水平下降(P<0.01),NO/ET比值升高(P<0.01);而对照组LAD血流和血浆NO和ET水平无明显变化(P>0.05)。Pearson线性相关分析显示,LAD血流流速的增加与血浆NO水平升高,ET水平下降,以及NO/ET比值升高显著相关(P<0.01)。结论银杏叶提取物可增加绝经后女性冠心病患者LAD血流,其机制可能与调节内源性血管活性物质有关。Objective To evaluate the effects of Ginkgo biloba extract on coronary blood flow and plasma nitric oxide(NO) and endothelin(ET) levels in postmenopausal women with coronary artery disease.Methods Sixty postmenopausal women with coronary artery disease were randomly assigned to Ginkgo biloba extract(n = 30) and control(n = 30) groups.At baseline and treatment,distal left anterior descending coronary artery(LAD)blood flow was assessed by transthoracic Doppler echocardiography ; Meanwhile, plasma NO and ET concentration were measured by nitrate reductase - dependent assay and radioimmunoassay, respectively. Results Ginkgo biloba extract treatment demonstrated a significant improvement in diastolic peak velocity ( P 〈 0.01 ), systolic peak velocity (P 〈 0.01 ) and diastolic time velocity integral ( P 〈 0.01 ), plasma NO was increased ( P 〈 0. 01), whereas ET was decreased (P 〈 0.01 ). NO/ET ratio was increased (P 〈 0.01 ). In contrast, these parameters of control group did not change (P 〉 0.05 ). A linear correlation was confirmed between the percentage change in LAD blood flow and NO, ET or NO/ET ratio following Ginkgo biloba extract treatment (P 〈 0.01 ). Conclusion Ginkgo biloba extract treatment in postmenopausal women with coronary artery disease led to an increase of LAD blood flow, which might at least partly related to positively regulate endogenous vasoactive substances.
