盐酸吡格列酮对煤工尘肺合并2型糖尿病β细胞功能的影响

摘  要:目的探索盐酸吡格列酮对煤工尘肺合并2型糖尿(T2DM)患者β细胞功能的影响。方法在煤工尘肺2型糖尿病和非煤工尘肺2型糖尿病原口服抗糖尿病药物方案不变的基础上联合盐酸吡格列酮治疗,连续观察12周,前后对比研究。结果联合方案治疗后血糖(空腹、餐后2h)、糖化血红蛋白(HbA1C)、胰岛素抵抗指数(Homa-IR)明显下降(P<0.05),胰岛素β细胞功能指数(Homa-β)、胰岛素敏感指数(Homa-ISI)较前明显升高(P<0.05)。非尘肺糖尿病组疗效较煤工尘肺并2型糖尿病组好,但组间比较,差异无统计学意义。结论盐酸吡格列酮能提高煤工尘肺合并2型糖尿病患者的Homa-β和Homa-ISI,降低其IR。疗效确切,值得临床推广使用。[ Objective] To explore the effect of pioglitazone hydrochloride on β-cell function of coal workers with pneumoconiosis and type 2 diabetes mellitus( T2DM). [ Methods] Anti-diabetic drug and pioglitazone hydroehloride were administrated orally to patients with coal worker's pneumoeoniosis and type 2 diabetes mellitus, and patients with non-coal worker's pneumoconiosis and type 2 diabetes mellitus for continuous 12 weeks, the results were compared before and after treatments. [ Results ] After combined treatment, blood glucose (FBG, PBG), HbA-1C and Homa-IR decreased significantly ( P 〈0.05), Homa-β and Homa-ISI increased significantly ( P 〈0. 05). The healing effect of non-pneumoconiosis group was better than that of pneumoconiosis group, but the difference was not significant. [ Conclusion] Pioglitazone hydrochloride can improve Homa-β, Homa-ISI, reduce IR of patients with coal worker's pneumoconiosis and type 2 diabetes mellitus. The treatment has exact effect and is worthy of clinical use.
